催产素引产时间对足月羊水过少孕妇妊娠结局的影响

摘　　要：目的探讨催产素引产时间对足月羊水过少孕妇围产儿及妊娠结局的影响。方法选取2016年1月至2017年12月在本院分娩的405例足月羊水过少孕妇作为研究对象。其中,累计滴催时间<8 h者为A组(109例),≥8 h且<12 h者为B组(54例),≥12 h且<16 h者为C组(43例),≥16 h者为D组(89例),从未使用催产素为对照组(110例)。结果(1)D组围产儿羊水污染、宫内窘迫发生率显著高于A、B、C组及对照组(均P<0.05);(2)D组产妇剖宫产、产后出血的比例与A组比较,差异有统计学意义(P<0.05);(3)D组产妇的总产程时间明显比(A+B+C)组及对照组要长,差异有统计学意义(P<0.05)。结论足月羊水过少孕妇催产素引产时间越长,特别是累计到达16 h以后,围产儿不良结局及不良妊娠结局发生率明显升高,对临床治疗有指导意义。催产素引产累计时间、总羊水量及孕周均是导致不良妊娠结局的危险因素。Objective To investigate the influence of cumulative time of labor induced by oxytocin on perinatal and pregnancy outcome of full-term pregnant women with oligohydramnios.Methods 405 cases of fullterm oligohydramnios pregnant women in our hospital from Jan.2016 to Dec.2017 were selected as study objects.Depending on the different accumulated time of oxytocin induction,the pregnant women were divided into group A(h<8 hours,n=109),group B(8≤h<12 hours,n=54),group C(12≤h<16 hours,n=43),group D(h>16 hours,n=89)and control group(never used oxytocin,n=110).Results(1)Group D’s incidence of amniotic fluid pollution and fetal distress was significantly higher than group A,B,C and control group(P<0.05).(2)Group D’s incidence of cesarean section and postpartum hemorrhage was significantly higher than group A(P<0.05).(3)Group D’s total labor time was longer than group(A+B+C)and control group(P<0.05).Conclusions The longer accumulated time of oxytocin induction used on full-term oligohydramnios pregnant women,the higher incidence of adverse perinatal and pregnancy outcomes could occur,especially after 16 hours.It has guiding significance for clinical treatment.The cumulative time of oxytocin inducing labor,total amniotic fluid volume and gestational weeks are risk factors for adverse pregnancy outcomes.
