经后颅凹立体定向置管血肿清除术治疗抗凝药物相关性小脑出血的应用价值

摘　　要：目的探讨经后颅凹立体定向置管血肿清除术治疗抗凝药物相关性小脑出血的应用价值。方法选择2018年1月-2019年10月在徐州市铜山区人民医院进行诊治的抗凝药物相关性小脑出血患者30例,所有患者都给予经后颅凹立体定向置管血肿清除术,记录与随访患者预后。结果所有患者均顺利完成手术,手术时间为(156.20±3.19)min,术后住院时间为(12.48±2.88)d,血肿清除率为(76.29±6.20)%。所有患者术后14d的NDS评分、APACHEⅡ评分都显著低于术前,GCS评分都显著高于术前,对比差异都有统计学意义(P<0.05)。所有患者随访至今,死亡3例,死亡率为10.0%。结论经后颅凹立体定向置管血肿清除术治疗抗凝药物相关性小脑出血具有很好的微创性,能显著改善患者预后。Objective To evaluate the clinical value of transcranial fossa stereotactic catheter hematoma removal in the treatment of anticoagulant related cerebellar hemorrhage.Methods 30 patients with anticoagulation-related cerebellar hemorrhage diagnosed and treated in People’s Hospital of Tongshan District,Xuzhou City from January 2018 to October 2019 were selected and were given transcranial fossa stereotaxic catheterization hematoma removal surgery.The prognosis of the patients was recorded and followed up.Results All patients successfully completed the operation,the operation time was(156.20±3.19)min,the postoperative hospitalization time was(12.48±2.88)d,the hematoma clearance rate was(76.29±6.20)%.After surgery 14 d,all of the patients’NDS score,APACHEⅡscore were significantly lower than before,GCS score was significantly higher than that of preoperative,the differences are statistically significant(P<0.05).All patients were followed up to date,and 3 patients were died,with a mortality rate of 10.0%.Conclusion Transcranial fossae stereotactic catheter hematoma removal is minimally invasive in the treatment of anticoagulant related cerebellar hemorrhage and can significantly improve the prognosis of patients.
