摘 要:目的观察麻杏二三汤对慢性阻塞性肺疾病(COPD)大鼠气道黏液高分泌的影响。方法36只SD大鼠随机分为正常组,模型组,阿奇霉素组,麻杏二三汤低、中、高剂量组,各6只。采用烟熏加气管内脂多糖滴入制备COPD模型。分别予以阿奇霉素及低、中、高剂量麻杏二三汤连续14 d灌胃干预,模型组予以等量0.9%氯化钠注射液。观察大鼠一般行为学表现;小动物肺功能仪检测大鼠肺功能指标;HE染色观察肺组织病理;酚红排痰实验检测气道黏液分泌;RT-PCR检测黏蛋白MUC5AC、MUC5B基因表达;Western blotting检测黏蛋白MUC5AC、MUC5B蛋白相对含量。结果与正常组比较,模型组大鼠肺组织HE染色符合COPD病理改变,肺阻力明显增加、肺顺应性明显下降(P<0.05),气道酚红排泌量及肺组织MUC5AC、MUC5B基因与蛋白明显增多(P<0.05)。与模型组比较,阿奇霉素组及麻杏二三汤中、高剂量组大鼠肺阻力明显减低(P<0.05),气道酚红排泌量及肺组织MUC5AC、MUC5B基因与蛋白明显减少(P<0.05)。结论下调MUC5AC、MUC5B基因与蛋白表达可能是麻杏二三汤治疗COPD气道黏液高分泌的作用机制之一。Objective:To investigate the effect of Maxing Ersan Decoction on airway mucus hypersecretion in COPD rats.Methods:36 SD rats were randomly divided into the normal group,the model group,Azithromycin group,low,medium and high dose groups of Maxing Ersan Decoction,6 rats in each.COPD model was prepared by smoking and intratracheal lipopolysaccharide.Azithromycin and low,medium and high dose Maxing Ersan Decoction were administered for 14 consecutive days,and the model group was given the same amount of normal saline.The general behavior of rats was observed.The pulmonary function of rats was measured with small animal pulmonary function instrument.HE staining was used to observe lung histopathology,phenol red sputum expelling test to detect airway mucus secretion,RT-PCR to detect MUC5AC and MUC5B gene expression,Western blotting to detect the relative content of MUC5AC and MUC5B.Results:Compared with the normal group,HE staining in the lung tissue of the model group was consistent with the pathological changes of COPD,and the lung resistance was significantly increased and lung compliance was significantly decreased(P<0.05).The amount of phenol red excretion in the airway and MUC5AC and MUC5B gene and protein in the lung tissue were significantly increased(P<0.05).Compared with the model group,the pulmonary resistance of the rats in Azithromycin group,the medium and high dose of Maxing Ersan Decoction groups was significantly reduced(P<0.05);the amount of phenol red excretion in the airway and the gene and protein of MUC5AC and MUC5B in the lung tissue were significantly reduced(P<0.05).Conclusion:Down regulation of MUC5AC and MUC5B gene and protein expression may be one of the mechanisms of Maxing Ersan Decoction on airway mucus hypersecretion in COPD.
