益气通腑法中药灌肠联合正压无创呼吸机治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭疗效观察

摘　　要：目的观察益气通腑法中药灌肠联合正压无创呼吸机治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的临床疗效。方法将80例COPD合并Ⅱ型呼吸衰竭患者按照随机数字表法分为2组。对照组40例予西医常规联合正压无创呼吸机治疗;治疗组40例在对照组治疗基础上联合益气通腑法中药灌肠治疗。2组均治疗1周,比较2组治疗前后动脉血气分析指标,包括酸碱度(p H值)、二氧化碳分压[p(CO2)]、氧分压[p(O2)]及动脉血氧饱和度(Sa O2)水平变化,以及撤机成功率、并发症发生率情况。结果治疗后2组p H值、p(O2)及Sa O2均升高(P<0.05),p(CO2)均降低(P<0.05),但组间比较差异均无统计学意义(P>0.05)。治疗后治疗组撤机成功率90%,对照组撤机成功率50%,治疗组撤机成功率高于对照组(P<0.05)。治疗组并发症发生率20.0%,对照组并发症发生率55.0%,治疗组并发症发生率低于对照组(P<0.05)。结论益气通腑法中药灌肠联合正压无创呼吸机治疗COPD合并Ⅱ型呼吸衰竭可缓解患者临床症状,改善动脉血气各项指标,降低患者对呼吸机的依赖性,进而提高撤机成功率,降低患者正压无创通气治疗并发症的发生率。Objective To observe the effect of Yiqi Tongfu method traditional Chinese medicine enema combined with positive pressure non-invasive ventilator in the treatment of chronic obstructive pulmonary disease(COPD)with type II respiratory failure.Methods 80 COPD patients with typeⅡrespiratory failure were divided into two groups according to the random number table method.40 cases in control group were treated by routine western medicine combined with positive pressure non-invasive ventilator.40 cases in treatment group were treated by Yiqi Tongfu method traditional Chinese medicine enema on the basis of the control group.Both groups were treated for one week,and the arterial blood gas analysis indexes before and after treatment were compared,including pH value,partial pressure of carbon dioxide[p(CO 2)],partial pressure of oxygen[p(O 2)]and arterial oxygen saturation(SaO 2),as well as the success rate of evacuation and the incidence of complications.Results After treatment,the pH,p(O 2)and SaO 2 of the two groups were increased(P<0.05),p(CO 2)was decreased,and there was no significant difference between the two groups(P>0.05).After treatment,the success rate of evacuation was 90%in treatment group and 50%in control group.The success rate of evacuation of the treatment group was better than that of the control group(P<0.05).The incidence of complications was 20.0%in treatment group and 55.0%in control group,the treatment group was lower than that of the control group(P<0.05).Conclusion Yiqi Tongfu method traditional Chinese medicine enema combined with positive pressure non-invasive ventilator can alleviate the clinical symptoms,improve the indexes of arterial blood gas,reduce the dependence of patients on ventilator,and then improve the success rate of ventilator withdrawal,reduce the incidence of complications of positive pressure non-invasive ventilation treatment.
