穴位重灸治疗变应性鼻炎临床研究

摘　　要：目的观察穴位重灸治疗变应性鼻炎(AR)的临床疗效。方法将180例AR患者按照随机数字表法分为2组。治疗组90例予穴位重灸治疗;对照组90例予布地奈德鼻喷雾剂治疗。2组均连续治疗4周,疗程结束后20周随访。比较2组治疗前后症状评分变化、鼻结膜炎生活质量问卷(RQLQ)评分变化,评价近期疗效和远期复发率。结果2组治疗后症状评分及总评分均较本组治疗前降低(P<0.05),且治疗组治疗后鼻塞症状改善优于对照组(P<0.05),对照组治疗后喷嚏症状改善优于治疗组(P<0.05)。2组治疗后RQLQ评分均较本组治疗前降低(P<0.05),且治疗组活动评分改善优于对照组(P<0.05),对照组非鼻/眼症状、实际问题、情感评分改善优于治疗组(P<0.05)。治疗组总有效率85.2%(75/88),对照组总有效率82.8%(72/88),2组近期疗效相当(P>0.05)。随访时,治疗组复发率72.5%(58/80),对照组复发率92.6%(75/81),对照组复发率高于治疗组(P<0.05)。结论穴位重灸治疗AR,能改善患者症状及生活质量,近期疗效与布地奈德鼻喷雾剂治疗相当,但远期疗效更优。Objective To observe the effect of heavey-dose moxibustion in the treatment of allergic rhinitis(AR).Methods 180 patients with AR were divided into two groups according to the random number table method.90 cases in treatment group were treated by heavey-dose moxibustion.90 cases in control group were treated by budesonide nasal spray.Both groups were treated for 4 weeks,and followeel up 20 weeks after treatment.The changes of symptom scores and quality of life questionnaire(RQLQ)were compared before and after treatment between two groups.The short-term efficacy and long-term recurrence rate of two groups were evaluated.Results The symptom scores and total scores of the two groups were lower than that of the group before treatment(P<0.05),and the improvement of nasal obstruction symptom in the treatment group was better than that in the control group(P<0.05),and the improvement of sneezing symptom in the control group was better than that in the treatment group(P<0.05).The RQLQ scores of the two groups after treatment were lower than that before treatment(P<0.05),and the improvement of activity score in the treatment group was better than that in the control group(P<0.05),and the improvement of non nose/eye symptoms,actual problems and emotional score in the control group was better than that in the treatment group(P<0.05).The total effective rate was 85.2%(75/88)in the treatment group and 82.8%(72/88)in the control group.The short-term effect of the two groups was similar(P>0.05).The recurrence rate was 72.5%(58/80)in the treatment group and 92.6%(75/81)in the control group.The recurrence rate in the control group was higher than that in the treatment group(P<0.05).Conclusion Heavey-dose moxibustion can improve the symptoms and quality of life of patients with AR.The short-term curative effect is similar to that of budesonide nasal spray,and the long-term curative effect is better than budesonide nasal spray.
