摘　　要：临床药师结合最新相关指南及文献,分析1例机械瓣置换术后伴房颤患者起搏器植入术围手术期抗凝方案的合理性和国际标准化比值(INR)升高的原因,权衡患者围手术期发生栓塞和出血的风险,参与制定个体化抗凝治疗方案,并在治疗期间密切监测INR变化,降低术后血栓栓塞和出血风险,保障患者用药安全、有效。According to the latest relevant guidelines and literature, clinical pharmacists analyzed the rationality of perioperative anticoagulation of the patient with atrial fibrillation after mechanical valve replacement during pacemaker implantation and the reasons for the increase in International standardized ratio(INR), and balance the risk of embolization and hemorrhage in patients during perioperative period and participate in the formulation of individualized anticoagulation regimens, and closely monitor the changes of INR during treatment, reduce the risk of postoperative thromboembolism and bleeding, and ensure safety and effectiveness of medication for patients.
