摘 要:目的了解武汉地区肺炎支原体IgM阳性患儿流行病学特征,对肺炎支原体感染的诊断、预防和有效控制提供科学的理论基础。方法收集2014年1月至2018年12月武汉大学人民医院儿科门诊和住院部以发热、咳嗽、流鼻涕等呼吸系统疾病进行就诊的患者。结果2014—2018年共收集46790例,MP-IgM阳性率为44.08%。男性MP-IgM阳性率为39.62%,而女性MP-IgM阳性率为51.00%,男性的MP-IgM阳性率低于女性(χ2=586.15,P<0.0001)。不同年龄段的阳性率的差异具有统计学意义,并且年龄≤3月的婴儿期的MP-IgM阳性率最低(χ2=4195.30,P<0.0001)。1岁之内的婴儿期在第1季和第2季的MP-IgM阳性率较第3季和第4季的阳性率高,差异有统计学意义(χ21d<年龄≤3月=25.05,χ23月<年龄≤6月=26.59,χ26月<年龄≤9月=26.18,P<0.0001),1~16岁各年龄段的患儿在第2季和第3季的MP-IgM阳性率较第1季和第4季的阳性率高,差异有统计学意义(χ21~3岁=17.59,χ24~6岁=28.92,χ27~16岁=11.43,P值<0.05)。结论MP-IgM阳性率与年龄、性别和季度具有一定关系。Objective To understand the epidemiological characteristics of children with positive IgM of mycoplasma pneumoniae(MP)in Wuhan,and to provide a scientific theoretical basis for the diagnosis,prevention and effective control of mycoplasma pneumoniae infection.Methods Patients with respiratory diseases such as fever,cough and runny nose from January 2014 to December 2018 in the Pediatric Outpatient Department and Inpatient Department of Wuhan University People's Hospital were included in the study.Results From 2014 to 2018,a total of 46,790 samples were collected,and the MP-IgM positive rate was 44.08%.The male MP-IgM positive rate was 39.62%,while the female MP-IgM positive rate was 51.00%.The MP-IgM positive rate in males was lower than that in females(χ2=586.15,P<0.0001).There were significant differences in the positive rate among different age groups,and the infection rate in infants aged≤3 months was the lowest(χ2=4195.30,P<0.0001).The MP-IgM positive rate in the first and second seasons of infancy within 1 year of age was higher than those in the third and fourth seasons,and the difference was statistically significant(χ21 days<age<3 months=25.05,χ23 months<age<6 months=26.59,χ26 months<age<9 months=26.18,P<0.0001).The MP-IgM positive rate of children aged 1-16 in the second and third seasons was higher than that in the first and fourth seasons,and the difference was statistically significant(χ21-3 years=17.59,χ24-6 years=28.92,χ27-16 years=11.43,P<0.05).Conclusion MP-IgM positive rate was related to age,gender and season.
