星状神经节阻滞联合针刺夹脊穴对急性期带状疱疹疼痛及炎症反应的影响

摘　　要：目的分析星状神经节阻滞与针刺夹脊穴联合用于急性期带状疱疹治疗的价值,研究联合治疗对患者急性期疼痛与炎症反应的影响。方法根据随机数字表法将2015年12月—2018年4月收治的80例急性期带状疱疹患者分为两组,每组40例。全部患者确诊后均接受常规治疗,对照组在此基础上给予患侧星状神经节阻滞,治疗组给予星状神经节阻滞联合针刺夹脊穴治疗,连续治疗2周后评估并对比两组治疗效果,观察记录患者各症状缓解情况,记录治疗期间不良反应发生情况。分别于治疗前、治疗1周时、治疗结束时,采用视觉模拟评分法(VAS)及皮损情况评分评价患者疼痛与皮损改善情况,检测并对比其治疗各时点血清白介素-6(IL-6)水平变化情况。结果治疗组治疗效果优于对照组,止疱、结痂、脱痂、疼痛缓解等用时均较对照组短,差异有统计学意义(P<0.05);治疗后各时点,两组患者VAS评分、皮损评分、血清炎性因子IL-6水平均较治疗前呈下降趋势,且治疗组下降速度高于对照组,两组组间、时点、组间·时点比较,差异有统计学意义(P<0.05);两组治疗期间均有轻微不良反应发生,差异比较无统计学意义(P>0.05)。结论星状神经节阻滞与针刺夹脊穴联合治疗急性期带状疱疹有利于缓解患者疼痛感,加速皮损止疱、结痂与脱痂,炎症反应明显减轻,不良反应少,安全可靠。Objective To analyze the value of stellate ganglion block combined with acupuncture on EX-B2 on acute herpes zoster,and to study its effect on pain and inflammatory response of patients.Methods 80 patients with acute herpes zoster admitted in the hospital from December,2015 to April,2018 were divided into the control group and the treatment group by random number table,with 40 patients in each group.All the patients received routine treatment after admission.The control group was additionally given stellate ganglion block,while the treatment group was given stellate ganglion block combined with on EX-B2 for 2 weeks.Clinical effect,alleviation of symptoms,adverse reactions were observed and compared.Before treatment,at the end of the 1st week,at the end of the treatment,Visual Analogue Scale(VAS)and skin lesion score were graded to evaluate pain and skin lesion condition;the change of serum interleukin-6(IL-6)level at different time points was compared.Results the treatment group had better effect,higher response rate and shorter time of herpes stopping and disappearing,incrustation,decrustation and pain relief than the control group,with significance(P<0.05).At all the time points after treatment,VAS score,skin lesion score,IL-6 level in both groups decreased.the treatment group decreased faster than the control group,with significant difference in groups,time points,group and time point(P<0.05).Mild adverse reactions appeared in both groups,with no significance(P>0.05).Conclusion Stellate ganglion block combined with acupuncture on EX-B2 can alleviate pain,accelerate the speed of herpes stopping and disappearing,incrustation and decrustation for acute herpes zoster,relieve inflammatory reactions,which is safe and reliable with less adverse reactions.
