糖尿病视网膜病变与糖尿病肾病发病相关性研究

摘　　要：目的:回顾性分析糖尿病患者临床中糖尿病视网膜病变(DR)与糖尿病肾病(DN)两种并发症发病情况及二者相关性。方法:纳入山东省立医院糖尿病患者116例,其中男性69例(59.48%),女性47例(40.52%);分析116例患者一般情况及并发症发生情况,并对DR及DN两种并发症发病情况进行相关性研究。结果:在纳入研究的116例糖尿病患者中,有34例同时伴有DR及DN,在伴有DR或DN患者中两种并发症同时存在患者占比50%。在血糖控制方面,出现DR或DN患者糖化血红蛋白及空腹血糖水平高于无DR或DN患者。两种并发症做关联性分析,Pearson列联系数为0.388。对其进行Kappa检验得出二者具有一致性,具有统计学意义(P=0.000)。结论:纳入研究的糖尿病患者中出现DR或DN并发症者血糖控制情况欠佳,在临床发病上DR与DN有关联性,这为临床糖尿病并发症的防治具有重要指导意义。Objective:To retrospectively analyze the pathogenesis and correlation of diabetic retinopathy (DR) and diabetic nephropathy (DN) in diabetic patients.Methods:A total of 116 cases of diabetes were included in the Shandong Provincial Hospital,among which 69 cases were males (59.48%) and 47 cases were females (40.52%).The complications of 116 patients were analyzed,and the correlation between the two complications of DR and DN was studied.Results:A total of 34 patients of the 116 diabetic patients included in the study,were accompanied by both DR and DN,and the proportion of patients with both complications was 50%.In terms of blood glucose control,HbA1c and fasting blood glucose levels of patients with DR or DN were higher than those of patients without DR or DN.Correlation analysis was performed for the two complications,and the Pearson correlation was 0.388.Kappa test on them showed that they were consistent,which had statistical significance (P=0.000).Conclusion:The blood glucose control of patients with DR or DN complications in diabetes patients included in our study is poor,and DR/DN is correlated with clinical morbidity,which is of great significance for the prevention and treatment of clinical diabetes complications.
