加减除湿胃苓汤治疗脾虚湿蕴型特应性皮炎的临床疗效观察

摘　　要：目的:客观评价加减除湿胃苓汤的有效性和安全性,为赵炳南教授经验方的临床推广应用提供相关依据。方法:选取2015年10月至2018年6月首都医科大学附属北京中医医院皮肤科收治的114例脾虚湿蕴型特应性皮炎患者为研究对象,随机分为治疗组和对照组各57例。对照组口服氯雷他定片,治疗组口服加减除湿胃苓汤,两组均外用维生素E乳膏治疗。观察两组特应性皮炎评分(SCORAD)、瘙痒视觉模拟量表(VAS)、皮肤病生活质量调查表(DLQI)评分变化、皮肤屏障功能(pH值、油脂、水分)改善以及不良反应。结果:经过8周治疗后,治疗组SCORAD、DLQI评分以及皮肤屏障指标pH值、油脂、水份改善明显优于对照组(P<0.05,P<0.01)。结论:加减除湿胃苓汤治疗脾虚湿蕴型特应性皮炎疗效可靠,可有效改善患者皮损症状以及皮肤屏障,提高患者生活质量,值得临床推广应用。Objective:To evaluate the efficacy and safety of Modified Chushi Weiling Decoction,and to provide basis for the clinical application of professor ZHAO Bing-nan’s experiential effective recipe.Methods:A total of 114 cases of atopic dermatitis with spleen deficiency and dampness deposition syndrome were selected from October 2015 to June 2018 in dermatological department of Beijing Hospital of Traditional Chinese Medicine,Capital Medical University.They were randomly divided into treatment group and control group with 57 cases respectively.The control group was treated with loratadine tablets and the treatment group was treated with Modified Chushi Weiling Decoction.Both groups were treated with vitamin E cream.The changes of SCORAD,DLQI and VAS scores,improvement of skin barrier function (pH value,lipids,humidity) and adverse reactions were compared between the two groups.Results:After 8 weeks treatment,the SCORAD,DLQI score,and the skin barrier indicators (pH value,lipids,humidity) of treatment group improved significantly compared with control group (P<0.05,P<0.01).Conclusion:Modified Chushi Weiling Decoction is effective and reliable in treating atopic dermatitis with spleen deficiency and dampness deposition syndrome,which is superior to western medicine.It can effectively improve the symptoms of skin lesions and skin barrier,improve the quality of life of patients,which is worthy of clinical application.
