补肾养血活血法改善冻融胚胎移植周期子宫内膜血流状态提高临床妊娠率的研究

摘　　要：目的:研究补肾养血活血法在改善冻融胚胎移植(FET)周期子宫内膜血流状态,提高临床妊娠率方面的作用。方法:以拟做FET的不孕症患者为研究对象,将入组患者随机分为中药组、中药+针刺组和对照组,经3个月治疗后,比较3组治疗前后子宫内膜血流状态及与妊娠结局的关系。结果:移植前中药、中药+针刺、对照组内膜内及内膜下均可见血流患者所占比重分别为55.77%、59.26%和42.00%,3组比较差异有统计学意义(P<0.05);各组间子宫内膜血流状态改善比较,差异无统计学意义,但有中药+针刺组优于中药组,中药组优于对照组的趋势。中药+针刺组的临床妊娠率为55.56%,中药组51.93%,对照组44.00%,组间比较差异无统计学意义;内膜内及内膜下均可见血流者临床妊娠率较高,差异具有统计学意义(P<0.05)。结论:补肾养血活血法可改善FET周期子宫内膜血流状态、提高临床妊娠率,中药联合针刺在改善子宫内膜血流方面具有更好的临床疗效。Objective:To investigate the effects of invigorating the kidney,nourishing and activating blood method on the improvement of endometrial blood flow status during a frozen-thawed embryo transfer (FET) cycle and the enhancement of clinical pregnancy rate.Methods:Infertile patients who underwent FET were recruited as study subjects and were randomly divided into three groups:traditional Chinese medicine (TCM) group TCM combined with acupuncture group,and control group.After 3 months of therapy,the relationships between pre-and post-treatment distributions of endometrial blood flow and pregnancy outcomes were compared among the three groups.Results:Before transfer,the percentage of patients presenting with blood flow within and under the endometrium in TCM group,TCM combined with acupuncture group,and control group was 55.77%,59.26%,and 42.00%,respectively,showing statistically significant differences (P<0.05).There were no statistically significant differences in clinical pregnancy rates among the three groups,but there is a trend that TCM combined with acupuncture group was better than TCM group,and TCM group was better than control group in improving endometrial blood flow.The clinical pregnancy rate was highest in TCM combined with acupuncture group (55.56%),second highest in TCM group (51.93%),and lowest in control group (44.00%),but there were no statistically significant differences in clinical pregnancy rates among the three groups.The clinical pregnancy rate was higher in patients who had blood flow both within and under the endometrium than those who did not,which was statistically significant (P<0.05).Conclusion:Invigorating the kidney,nourishing and activating blood method improves endometrial blood flow during the FET cycle and enhances the clinical pregnancy rate.A combination of TCM and acupuncture showed a better clinical therapeutic efficacy for improving endometrial blood flow than TCM alone.
