摘 要:目的:探讨针刀干预对颈椎病兔颈后伸肌细胞PI3K/Akt信号通路的影响。方法:40只新西兰雄性兔按随机数字表随机分为空白组、模型组、针刀组、电针组,每组10只,空白组不做干预处理,其余建立颈椎病动物模型,持续12周。造模成功后针刀组于兔颈肌硬结、条索以及棘突等处行针刀干预,每周1次,共3次;电针组于兔颈肌两侧“天柱”“颈百劳”“大杼”穴处行电针干预,隔天治疗1次,每周3次,共3周。干预结束后1周取兔颈后两侧后伸肌,采用Real-time PCR法检测PI3K mRNA水平,Western blot法检测PI3K、Akt和P-Akt蛋白相对表达水平。结果:与空白组比较,模型组PI3K mRNA、蛋白及P-Akt蛋白表达显著下调(P<0.05,P<0.01);与模型组比较,针刀组PI3K mRNA、蛋白表达及P-Akt蛋白表达显著上调(P<0.01),电针组P-Akt蛋白表达显著上调(P<0.05);与电针组比较,针刀组PI3K mRNA、蛋白表达及P-Akt蛋白表达显著上调(P<0.05);Akt蛋白表达变化在各组之间的差异无统计学意义。结论:针刀治疗颈椎病具有一定优势,作用机制可能是针刀通过PI3K/Akt信号通路调节颈后伸肌细胞的自噬、凋亡平衡,这可能是针刀修复劳损颈肌的潜在靶点。Objective:To investigate the effects of acupotomy on PI3K/Akt signaling pathway of posterior cervical extensor muscles cells in cervical spondylosis(CS)rabbits.Methods:Forty New Zealand male rabbits were randomly divided into normal control,model,acupotomy,and electro-acupuncture(EA)groups(10 rats in each group).No intervention was given in control group,and the other groups established the CS animal model for 12 weeks.After the model was successfully made,acupotomy was performed at the point of indurate,cord-like mass and spinous process,once a week for three weeks.For EA group,EA was applied to bilateral‘Tianzhu’(BL 10),‘Jingbailao’(EX-HN 15),‘Dazhu’(BL 11)of the rabbit neck muscle,once daily,three times a week for 3 weeks.One week after the end of intervention,the posterior extensor muscles were taken from bilateral rabbits’neck muscles.The expression level of PI3K mRNA was detected by Real-time PCR method,and the expression of PI3K,Akt and P-Akt proteins were detected by Western blot.Results:Compared with control group,the expression level of PI3K mRNA,PI3K protein and P-Akt protein in model group were decreased(P<0.05,P<0.01);Compared with model group,PI3K mRNA,PI3K protein and P-Akt protein in acupotomy group were increased(P<0.01).the expression level of P-Akt protein in EA group were increased(P<0.05);Compared with EA group,the expression level of PI3K mRNA,PI3K protein and P-Akt protein in acupotomy group were increased(P<0.05).There was no significant difference in the expression of Akt protein among all groups.Conclusion:Acupotomy has certain advantages in the treatment of cervical spondylopathy,and the mechanism may be that acupotomy regulates the autophagy and apoptosis balance of cervical muscle cells through PI3K/Akt signaling pathway,which may be a potential target for acupotomy to repair cervical muscle strain.
