中药配合针刺、按摩、发声训练治疗功能性发声疲劳的临床观察

摘　　要：目的:评价补中益气汤合百合固金汤配合针刺、按摩、发声训练治疗功能性发声疲劳的疗效。方法:采用自身前后对照研究,纳入功能性发声疲劳患者11例,给予补中益气汤合百合固金汤中药汤剂口服配合针刺、按摩、发声训练治疗,共治疗4周,观察治疗前后嗓音障碍指数(VHI-10)量表、GRB、咽喉反流症状量表(RSI)及反流体征量表(RFS)评分,最长声时(MP T)、嗓音障碍严重指数(DSI)、声门闭合商(CQ)、嗓音疲劳度测试(VLT)分级结果。结果:11例患者治疗后RSI、MPT、DSI、CQ较治疗前均有显著改善(P<0.05),且VLT与RFS相关性较高。治疗后VLT在第1、4、5、6、7、8、10分钟较治疗前显著降低。结论:中药配合针刺、按摩、发声训练治疗功能性发声疲劳疗效确切;RSI、MPT、DSI、CQ可作为功能性发声疲劳病情判断及疗效评价的敏感指标。Objective:To evaluate the effects of Buzhong Yiqi Decoction and Baihe Gujin Decoction combined with acupuncture,massage and vocal training in the treatment of functional vocal fatigue(FVF).Methods:Eleven patients with FVF were included in the self-controlled study,and treated with Buzhong Yiqi Decoction and Baihe Gujin Decoction combined with acupuncture and moxibustion,massage and vocal training.The voice handicap index-10(VHI-10),grade roughness breathiness(GRB),reflux symptom index(RSI)and reflux finding score(RFS)scale,maximum phonation time(MPT),dysphonia severity index(DSI),closure quotient(CQ)and vocal fatigue test(VLT)results at 4 weeks after treatment was compared with those before treatment.Results:RSI,MPT,DSI and CQ were improved in 11 patients with FVF after treatment(P<0.05),and VLT was highly correlated with RFS.After treatment,the VLT at the time point of 1,4,5,6,7,8 and 10 minutes were significantly lower than the VLT before.Conclusion:Chinese materia medica combined with acupuncture and moxibustion,massage and vocal training is effective in treating FVF.RSI,MPT,DSI and CQ can be used as sensitive indicators for judging the condition of functional vocal fatigue and evaluating the curative effect.
