摘 要:目的探讨脓毒症患者血清白介素(IL)-23、IL-17水平及转化生长因子-β(TGF-β)/Smad信号通路作用.方法选择2016年12月-2018年12月北京世纪坛医院收治的脓毒症患者97例,其中脓毒症组29例,严重脓毒症组37例,脓毒症休克组31例.另选择同期医院健康体检者80名作为对照组.分析急性生理与慢性健康状况评分II(APACHE II)和序贯性器官衰竭评分(SOFA),采用酶联免疫吸附法测定血清IL-23、IL-17、TGF-β1、Smad2和Smad7含量.结果脓毒症组、严重脓毒症组和脓毒症休克组APACHE II和SOFA高于对照组,严重脓毒症组和脓毒症休克组高于脓毒症组,且脓毒症休克组高于严重脓毒症组(P<0.05).脓毒症组、严重脓毒症组和脓毒症休克组血清IL-23和IL-17水平高于对照组,严重脓毒症组和脓毒症休克组高于脓毒症组,且脓毒症休克组高于严重脓毒症组(P<0.05).脓毒症组、严重脓毒症组和脓毒症休克组血清TGF-β1、Smad2和Smad7水平高于对照组,严重脓毒症组和脓毒症休克组高于脓毒症组,且脓毒症休克组高于严重脓毒症组(均P<0.05).结论血清IL-23和IL-17及TGF-β/Smad信号通路可能参与脓毒症发生、发展,且与病情进展密切相关,检测有助于诊断和指导治疗.OBJECTIVE To explore the levels of serum interleukin-23(IL-23) and interleukin-17(IL-17) and role of transforming growth factor-β/Smad signaling pathway in sepsis patients. METHODS A total of 97 patients with sepsis who were treated in Beijing Century Temple Hospital Affiliated to Capital Medical University from Dec 2016 to Dec 2018 were enrolled in the study, 29 patients were assigned as the sepsis group, 37 patients were assigned as the severe sepsis group, and 31 patients were assigned as the septic shock group;meanwhile, 80 people who received physical examination were set as the control group. The Acute Physiology and Chronic Health Evaluation Score II(APACHE II) and Sequential Organ Failure Score(SOFA) were observed, and the levels of serum IL-23, IL-17, TGF-β1, Smad2 and Smad7 were determined by means of enzyme-linked immunosorbent assay. RESULTS The APACHE II and SOFA of the sepsis group, the severe sepsis group and the septic shock group were significantiy higher than those of the control group, the APACHE II and SOFA of the severe sepsis group and the septic shock group were significantiy higher than those of the sepsis group, and the APACHE II and SOFA of the septic shock group were significantiy higher than those of the severe sepsis group( P<0.05). The levels of serum IL-23 and IL-17 of the sepsis group, the severe sepsis group and the septic shock group were significantiy higher than those of the control group, the levels of serum IL-23 and IL-17 of the severe sepsis group and the septic shock group were significantiy higher than those of the sepsis group, and the levels of serum IL-23 and IL-17 of the septic shock group were higher than those of the severe sepsis group(P<0.05). The levels of TGF-β1, Smad2 and Smad7 of the sepsis group, the severe sepsis group and the septic shock group were significantiy higher than those of the control group, the levels of TGF-β1, Smad2 and Smad7 of the severe sepsis group and the septic shock group were significantiy higher than those of the sepsis group, and
