摘　　要：目的消除抗药物-药物复合物引起的不规则抗体筛查假阳性结果。方法对微柱凝胶法不规则抗体筛查阳性的29例患者标本,采用盐水介质法、吸收试验和二硫苏糖醇(DTT)法平行检测,将初步确定为抗药物-药物复合物引起的假阳性标本置于4℃冰箱3 d后,再用微柱凝胶法、吸收试验和DTT法平行检测确认抗药物-药物复合物。结果 29例微柱凝胶法不规则抗体筛查阳性的标本中,因抗药物-药物复合物引起的假阳性占62.07%(18/29):置4℃冰箱3 d前后吸收试验和DTT法检测均阳性14例,均为吸收试验阴性和DTT法检测阳性4例。其余11例吸收试验阳性、DTT法阴性标本,置4℃冰箱3 d微柱凝胶法、吸收试验及DTT法检测均为阴性。结论 DTT能够降解抗药物-药物复合物,消除由后者引起的不规则抗体筛查假阳性。Objective To avoid the false positive results of irregular antibodies screening caused by anti-drug-drug complex. Methods 29 samples which initially had positive results for irregular antibodies screening with the microcolumn gel test were retested in parallel by saline medium method, absorption test and DTT. The false positive samples caused by anti-drug-drug complex were placed in the refrigerator at 4° C for 3 d, and then microcolulmn gel test, absorption test and DTT were employed to retest and confirm the anti-drug-drug complex parallelly. Results Among the 29 initially positive samples, the false positive results owing to the presence of anti-drug-drug complex accounted for 62.07%(18/29). After 3-day storage in freezer at 4℃, 14 samples remained positive with absorption test and DTT, and 4 samples remained positive with DTT and negative with absorption test. As to the remaining 11 samples, which were initially positive with absorption test and negative with DTT, all turned into negative with microcolumn gel test, absorption test and DDT after 3-day storage in 4℃ freezer. Conclusion The false positive results of irregular antibody screening owing to the presence of anti-drug-drug complex can be avoided using DDT test, as DDT can degrade it.
