自制导管引流在经尿道膀胱结石激光碎石中的应用研究

摘　　要：目的评价使用自制导管持续引流在经尿道膀胱结石激光碎石术中的临床效果。方法分析医院自2014年6月—2018年11月,通过使用间断冲吸水方式及使用自制导管持续引流(改良)方式各20例均行经尿道膀胱结石激光碎石术,术后分析其术中用水量,出血量,手术时间及膀胱黏膜损伤等数据来评价该改良方法是否有优越性。结果改良组从冲水量、碎石时间及膀胱损伤等方面均优于普通组。结论采用自制输尿管导管持续引流在经尿道膀胱镜下膀胱结石激光碎石术中可持续冲洗及引流,保持膀胱视野清晰,碎石过程持续进行明显提高手术效率,膀胱内结石活动度较小,膀胱损伤较少,碎石成功率高,此改良方法效果确切,安全、并发症少。Objective To evaluate the clinical effect of continuous drainage of self-made catheter in transurethral calculi with laser lithotripsy.Methods From June 2014 to November 2018,all patients underwent transurethral calculi with laser lithotripsy by intermittent irrigation and continuous drainage(improvement).Data on water consumption,bleeding volume,operative time,and bladder mucosal damage were used to evaluate whether the improved method was superior.Results The improvement group was superior to the control group in terms of flushing volume,lithotripsy time and bladder injury.Conclusion Continuous drainage of self-made ureteral catheter can be used for continuous irrigation and drainage in transurethral cystoscopy laser lithotripsy,which keeps the bladder vision clear.The gravel process continues to improve the operation efficiency,and the bladder activity is small.Less damage,high success rate of crushed stone,this improved method is effective,safe,less complications.
