拼接式头面部枕在脊柱后路手术体位中的应用观察

摘　　要：目的:探讨拼接式头面部枕在脊柱后路手术体位中的应用效果。方法:随机数字表法将80例脊柱后路手术患者分为对照组和观察组各40例,观察组患者采用拼接式头面部枕完成头面部体位摆放,对照组采用传统的头圈和软垫完成。比较两组患者体位摆放所需的时间、医护人员满意度情况、术后头面部皮肤受压状况(包括皮肤压痕、红斑、水疱、破损等)及气管导管移位、扭曲发生率。结果:观察组体位安置平均耗时(8.73±0.83)min,对照组平均(12.50±1.08)min;观察组医护人员满意度明显高于对照组;观察组患者发生头面部皮肤压痕3例,无气管导管移位及扭曲的发生,对照组发生头面部皮肤受压23例,气管导管移位6例,气管导管扭曲7例。两组比较,差异均有统计学意义(P<0.05)。结论:在脊柱后路手术中应用拼接式头面部枕,可有效缩短患者摆放体位时间,减少头面部皮肤并发症的发生及气管导管移位、扭曲的发生率,达到提高效率及安全性的目的。Objective:To explore the application effect of spliced head and face pillow in posterior spinal surgical position.Methods:80 patients with posterior spinal surgery were randomly divided into control group and observation group,40 cases in each group.The observation group were treated by spliced head and facial pillow,the control group used the traditional head ring and cushion.The time of prone position setting,satisfaction of medical staff,skin compression(including skin indentation,erythema,blisters,damage,etc.)and the incidence of displacement and distortion of tracheal catheter between the two groups were compared.Results:The time of prone position setting was(8.73±0.83)min in the observation group and(12.50±1.08)min in the control group;The satisfaction of the medical and nursing staff in the observation group was significantly higher than that of the control group;There were 3 cases with skin compression in the observation group and no tracheal catheter displacement and distortion occurred;There were 23 cases with skin compression in the control group,6 cases of tracheal catheter displacement and 7 cases of tracheal tube distortion(P<0.05).Conclusions:In the posterior spinal surgery,the application of the spliced head and face pillow can effectively shorten the placement time of patients,reduce the incidence of skin complications and tracheal catheter displacement and distortion,and improve the efficiency and safety.
