养血清脑颗粒对血管内皮功能影响的Meta分析

摘　　要：目的系统评价养血清脑颗粒改善血管内皮功能的效果。方法系统全面地检索中国知网(CNKI)、万方(WanFang)、中国生物医学文献数据库(CBM)、维普(VIP)、PubMed、EMbase及Cochrane图书馆,检索时间为2008年1月—2019年8月,采用“Cochrane偏倚风险评估”工具对所纳入的文献进行质量评价,并对纳入的文献使用RevMan 5.3软件进行Meta定量分析。结果共纳入17项研究,总样本量1542例,试验组771例,对照组771例。Meta分析结果显示:与对照组(常规综合治疗)比较,试验组(养血清脑颗粒+常规综合治疗)能显著降低纤维蛋白原含量[MD=-0.77,95%CI(-0.92,-0.62),P<0.00001],降低内皮素-1水平[SMD=-1.76,95%CI(-2.66,-0.85),P=0.0002],提高内皮依赖性血管舒张功能[MD=4.40,95%CI(3.67,5.12),P<0.00001],提升一氧化氮水平[MD=9.52,95%CI(4.79,14.25),P<0.0001],降低颈动脉内膜中膜厚度[MD=-0.27,95%CI(-0.34,-0.21),P<0.00001],降低低密度脂蛋白胆固醇含量[MD=-0.50,95%CI(-0.64,-0.35),P<0.00001]。结论基于现有数据和方法表明,养血清脑颗粒联合西医常规治疗可改善血管内皮功能,改善病情的进展与预后,无严重的不良反应。Objective To explore the effect of Yangxue Qingnao Granule in vascular endothelial function.Methods CNKI,WanFang,CBM,VIP,PubMed,EMbase,and Cochrane Libraries were systematically and comprehensively searched.The retrieval time was from January 2008 to August 2019."Cochrane Bias Risk Assessment"tool was used to evaluate the quality of the included literature,and RevMan5.3 software was used for meta-quantitative analysis of the included literature.Results The Meta-analysis demonstrated that a total of 17 articles were included in the study,with 1542 cases,771 cases in experimental group,771 cases in control group.Compared with control group(routine comprehensive treatment),the experimental group(Yangxue Qingnao Granule+routine comprehensive treatment)could significantly reduce the content of fibrinogen[MD=-0.77,95%CI(-0.92,-0.62),P<0.00001],and the level of endothelin-1[SMD=-1.76,95%CI(-2.66,-0.85),P=0.0002],enhance endothelium-dependent vasodilation[MD=4.40,95%CI(3.67,5.12),P<0.00001]and nitric oxide[MD=9.52,95%CI(4.79,14.25),P<0.0001],reduce carotid intima-media thickness[MD=-0.27,95%CI(-0.34,-0.21),P<0.00001],decrease LDL cholesterol content[MD=-0.50,95%CI(-0.64,-0.35),P<0.00001].Conclusion Based on the existing data and methods,Yangxue Qingnao Granule combined with routine western medicine treatment can improve vascular endothelial function,progress and prognosis of disease,and no serious adverse reactions occurred.
