卵圆钳辅助穿针技术与开放Kessler缝合在急性跟腱断裂中的临床疗效研究

摘　　要：目的探讨卵圆钳辅助穿针技术治疗急性闭合性跟腱断裂的临床疗效及可行性。方法回顾性分析2013年1月至2019年6月我院43例接受手术治疗的急性闭合性跟腱断裂患者,其中17例采用卵圆钳辅助穿针技术(微创组),26例采用开放Kessler缝合治疗(开放组),观察并比较两组患者的手术疗效。结果卵圆钳辅助微创组在手术时间、术中出血量、住院时间均短于开放组,且差异有统计学意义(P<0.05)。微创组并未出现术后并发症,而开放组出现的术后伤口感染、延迟愈合、皮缘坏死和再次断裂发生率分别为:11.54%、15.38%、15.38%和11.54%。结论卵圆钳辅助穿针技术是一种安全可靠的治疗跟腱断裂的方法,与开放手术相比,能够减少软组织并发症的发生,并可减少手术时长、住院天数及康复时间;提高了患者的生活质量并降低了社会成本。Objective To investigate the clinical efficacy and feasibility of forceps assisted minimally invasive technology in the treatment of acute closed Achilles tendon rupture.Methods A retrospective analysis was carried out in 43 patients with acute closed rupture of Achilles tendon who underwent surgery in our hospital from January 2013 to June 2019,of which 17 cases were treated with forceps assisted puncture and 26 cases with open Kessler suture.The clinical efficacy were observed and compared between the two groups.Results Operation time,intraoperative blood loss and hospital stay in the forceps assisted puncture group were less than that of the open group,and the difference was statistically significant(P<0.05).There were no postoperative complications in the minimally invasive group,while the incidences of postoperative wound infection,delayed healing,marginal necrosis,and re-rupture in the open group were 11.54%,15.38%,15.38%and 11.54%respectively.Conclusion Forceps assisted minimally invasive treatment is a safe and reliable method for the treatment of Achilles tendon rupture.Compared with open surgery,it can reduce the occurrence of soft tissue complications and reduce operation time,hospital stay and recovery time.It improves the quality of life and reduces the social cost.
