龙氏正骨手法联合高压氧治疗颈源性头痛的疗效观察

摘　　要：目的:探究龙氏正骨手法联合高压氧(hyperbaric oxygen,HBO)治疗颈源性头痛(cervicogenic headache,CEH)的临床疗效。方法:选取2017年1月至2019年5月山西医科大学第二医院康复科接受治疗的CEH患者80例。采取随机数字表法分观察组和对照组,每组40例。对照组患者给予龙氏正骨手法治疗,观察组在对照组治疗的基础上联合HBO治疗,共治疗10 d。观察2组患者的血液流变学、头痛指数、颈椎活动度(range of motion, ROM)评分以及视觉模拟评分(visual analogy score, VAS)。结果:治疗10 d后,观察组患者总有效率(95.0%)明显高于对照组(85.0%),差异有统计学意义( χ2=3.89, P<0.05);2组患者每日头痛指数,VAS、ROM评分,细胞压积,血浆黏度,纤维蛋白原,血清前列腺素F2α和血浆血管加压素水平均低于治疗前,且观察组治疗后均低于对照组,差异均有统计学意义( P<0.05或 P<0.01)。 结论:龙氏正骨手法联合HBO治疗颈源性头痛具有明显疗效,值得临床推广应用。Objective To explore the clinical effect of Long′s bone setting manipulation combined with hyperbaric oxygen(HBO)on cervicogenic headache(CEH).Methods A total of 80 patients with CEH,treated in the Department of Rehabilitation of the Second Hospital of Shanxi Medical University from January 2018 to May 2019 were selected and randomly divided into the observation group and the control group according to the random number table method,with each group consisting of 40 patients.The patients in the control group were treated with Long′s bone setting manipulation,while those in the observation group were treated with HBO combined with the same manipulation of the control group.The hemorheology indexes,headache index,range of motion(ROM)and visual analogy scoring(VAS)of the two groups before and after treatment were recorded and compared.Results After 10 days treatment,the total effective rate of the observation group(95.0%)was significantly higher than that of the control group(85.0%);the difference was statistically significant(P<0.05).The daily headache index,VAS and ROM scores,packed cell volume,plasma viscosity,fibrinogen,serum prostaglandin F2α,and arginine vasopressin of both two groups were all significantly lower than those before treatment;and those of the observation group after treatment were lower than those of the control group;these differences were statistically significant(P<0.05,P<0.01).Conclusion Long′s bone setting manipulation combined with HBO is effective in the treatment of CEH.This treatment,therefore,is worthy of popularization and application in clinic.
