超声联合注气法在意识障碍患者鼻胃管置入中的应用及效果评价

摘　　要：目的探讨超声联合注气法在意识障碍患者鼻胃管置入中应用的安全性和有效性。方法按照时间先后顺序,将我院重症医学科2017年2-7月需置入鼻胃管的意识障碍患者104例,分为两组,对照组42例采用常规超声鼻胃管置入法,观察组62例采用超声联合注气鼻胃管置入法。比较两组患者的置管成功率、鼻胃管显影率及超声探查鼻胃管深度,胃管盘在口腔和误入气道的发生率。结果观察组的置管成功率和鼻胃管显影率均高于对照组,差异有统计学意义(P<0.05)。两组超声探查胃管深度、胃管盘在口腔及误入气道的发生率差异无统计学意义(P>0.05)。结论超声联合注气法置入鼻胃管提高超声显影率,可有效的快速判断胃肠管位置,避免严重并发症的发生,提高意识障碍患者中鼻胃管置入成功率,值得在临床上推广使用。Objective To study the safety and effectiveness of ultrasound combined with gas injection in nasogastric tube placement in patients with consciousness disorder.Methods According to the sequence of time,104 patients with consciousness disorder who needed to be inserted nasogastric tube in the department of critical care medicine of a graded A hospital in Beijing from February to July 2017 were divided into two groups.The control group was treated with routine ultrasound nasogastric tube placement,while the experimental group was treated with ultrasound combined with gas injection nasogastric tube placement.The success rate of nasogastric tube placement,nasogastric tube development rate,the depth of nasogastric tube detected by ultrasound,and the incidence of gastric tube disc in oral cavity and mistaken airway were compared between the two groups.Results The success rate of intubation and the development rate of nasogastric tube in the experimental group were higher than those in the control group,and there was statistically significant.There was no significant difference between the two groups in the depth of ultrasound examination,the incidence of disc in oral cavity and mistaken airway.Conclusion Ultrasound combined with gas injection can improve the development rate of ultrasound,effectively and quickly determine the location of gastrointestinal tract,avoid serious complications,and improve the success rate of nasogastric tube placement in patients with disturbance of consciousness.It is worth popularizing in clinic.
