双Endobutton带袢钢板重建喙锁韧带治疗RockwoodⅢ-Ⅴ型肩锁关节损伤的临床研究

摘　　要：目的:评价双Endobutton带袢钢板重建喙锁韧带治疗RockwoodⅢ-Ⅴ型肩锁关节损伤的临床安全性和有效性。方法:以2015年05月至2017年05月诊断的RockwoodⅢ-Ⅴ型肩锁关节损伤患者60例为研究对象,锁骨钩钢板固定肩锁关节治疗的35例为对照组,采用双Endobutton带袢钢板治疗的25例为观察组;比较两组的平均手术时间、术中出血量和术后康复时间,围术期并发症,术后12 h、24 h和72 h的疼痛VAS评分,随访1月、3月和6月的肩关节功能Constant评分。结果:两组平均手术时间无明显差异(P>0.05);观察组术中出血量、术后康复时间、围术期并发症均明显少于对照组(P均<0.05)。观察组术后12 h、24 h和72 h的疼痛VAS评分均明显小于对照组(P<均0.05)。随访1月、3月和6月的肩关节功能Constant评分及6月的总优良率显著高于对照组(P<0.05)。结论:双Endobutton带袢钢板重建喙锁韧带治疗RockwoodⅢ-Ⅴ型肩锁关节损伤有较好的临床安全性和有效性。Objective:To evulate clinical safety and effectiveness of double Endobutton band loop plate for acromioclavicular ligament reconstruction in treatment of types Rockwood Ⅲ toⅤ acromioclavicular joint injury.Methods:A total of 60 consecutives suitable for the experiment from May 2015 to May 2017 were enrolled,35 cases of which received clavicular hook plate for acromioclavicular joint fixation in control group and other 25 cases adopted double Endobutton band loop plate in observation group,then to compare differences of mean operative time,intraoperative blood loss,postoperative recovery time,and perioperative complications,pain VAS scores 12 h,24 h and 72 h after operation,shoulder function Constant scores one month,three months and six months after follow?up.Results:The mean operative time in the two groups were no statistical differences(P>0.05).The blood loss,recovery time and complications in observation group were significantly less than those in the control group(all P<0.05).The pain VAS scores after 12 h,24 h and 72 h in observation group were significantly lower than those in the control group(all P<0.05).The shoulder function Constant scores after one month,three and six months and the total excellent rate after six months in observation group were significantly higher than those in the control group(all P<0.05).Conclusion:It is better clinical safety and effectiveness with double Endobutton band loop plate for acromioclavicular ligament reconstruction in treatment of types Rockwood Ⅲ toⅤacromioclavicular joint injury.
