超声引导下腰丛联合坐骨神经阻滞应用于老年下肢骨科手术的研究

摘　　要：目的:分析超声引导下腰丛联合坐骨神经阻滞应用于老年下肢骨科手术的价值.方法:选择2017年6月-2018年6月来我院接受下肢骨科手术的老年患者为研究对象,将其分为2组,每组90例.对照组受试者接受常规化腰丛坐骨神经阻滞麻醉术,观察组实施超声引导下腰丛联合坐骨神经阻滞,分析结果.结果:相较于对照组,观察组受试者运动以及感觉神经组织起效时间、维持时间明显更优,P<0.05.治疗前,2组受试者的VAS评分无明显差别,P>0.05.治疗后,观察组的2小时、4小时、6小时、8小时VAS分数明显比对照组低,P<0.05.结论:对于需要接受下肢骨科手术的老年患者来讲,开展超声引导下腰丛坐骨神经麻醉阻滞术加以处理,不但能够在根本上改善受试者各项生理指标,同时也可继续提升麻醉最优良率,进而缓解患者苦楚和不适感,因此值得进一步在临床中推广应用.Objective:To analyze the value of ultrasound-guided lower lumbar plexus combined with sciatic nerve block in the appli-cation of lower limb orthopedic surgery in the elderly.Methods:Elderly patients admitted to our hospital for lower limb orthopedic surgery from June 2017 to June 2018 were selected as the study subjects and divided into two groups,90 cases in each group.The subjects in the control group received routine anesthesia for lumbar plexus sciatic nerve block,and the observation group received ultrasound-guided lumbar plexus combined with sciatic nerve block,and the results were analyzed.Results:compared with the control group,the onset and maintenance time of movement and sensory nerve tissue in the observation group were significantly better,p<0.05.Before treatment,there was no significant difference in VAS score between the two groups(P>0.05).After treatment,the scores of 2h,4h,6h and 8hVAS in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:for elderly patients with lower extremity orthopedic surgery,to carry out under the guidance of ultrasound from the sciatic nerve anesthetic technique process-ing,not only can fundamentally improve the subjects the physiological indexes,the most excellent and at the same time also can continue to ascend anesthesia,thereby alleviating patients suffering and discomfort,so worthy of further popularization and application in clinical.
