述情障碍在不孕症患者反刍思维与抑郁间的中介作用

摘　　要：目的探究不孕症患者述情障碍、反刍思维、抑郁的现状以及三者之间的关系。方法采用横断面现状调查,选择2018年9—12月于南京医科大学第一附属医院生殖中心就诊的229例不孕症患者进行现场问卷填写,以t检验、方差分析、路径分析等方法对资料进行分析。结果不孕症患者抑郁得分(13.17±8.66)分、述情障碍得分(51.62±10.91)分、反刍思维得分(18.28±10.65)分,三者之间呈正相关(r值为0.442、0.551、0.569,均P<0.05),述情障碍在不孕症患者反刍思维与抑郁间起中介作用,其间接效应为0.0264,占总效应的24.81%。结论不孕症患者抑郁现状不容乐观,反刍思维通过述情障碍对其造成了很大的影响,患者内心的情感不能有效抒发,容易形成负性应激情绪,从而导致抑郁的发生,医护人员、家属应当重视患者内心想法和反刍思维,及时倾听、疏导,促进患者身心康复。Objective To explore the status quo of alexithymia,ruminant thinking and depression in infertility patients and their relationship.Methods A cross-sectional survey was conducted among infertile patients who visited the reproductive center of the First Affiliated Hospital of Nanjing Medical University from September to December 2018.The data were analyzed by descriptive analysis,t-test,variance analysis and path analysis.Results There was a moderate positive correlation between depression score(13.17±8.66),alexithymia score(51.62±10.91)and ruminant thinking score(18.28±10.65)in infertile patients(r values were 0.442,0.551,0.569,all P<0.05).Alexithymia played a mediating role between ruminant thinking and depression in infertile patients,and its indirect effect was 0.0264,accounting for the total effect 24.81%.Conclusions The depression status of infertility patients is not optimistic.Ruminant thinking has a great impact on infertility patients through alexithymia.The inner feelings of infertility patients can not be effectively expressed.It is easy to form negative emotional stress,which leads to depression.Medical staff and family members should pay attention to the ruminant thinking of patients'inner thoughts and timely dumping.Hearing and counseling can promote the physical and mental recovery of patients.
