疮灵液外治对浆细胞性乳腺炎肿块的影响

摘　　要：目的观察疮灵液外用对缩小浆细胞乳腺炎(PCM)局部肿块的影响。方法将40例需要换药的PCM患者,按随机数字表法分组为试验组和对照组,每组20例。所有患者切开排脓或脓肿自溃后,试验组采用疮灵液30 mL冲洗,疮灵液纱条引流;对照组生理盐水冲洗,生理盐水纱条引流。1周2次,共4周。观察两组治疗前(第1次换药前),治疗中(第14天),治疗后(第28天)肿块大小,患者疼痛评分,换药时取局部组织,采用ELISA法检测IL-1β、IL-2、IL-6、γ-干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)的水平。结果与本组治疗前比较,两组肿块均明显缩小(P<0.05),且治疗28天时试验组缩小程度较对照组更明显(P<0.05)。两组治疗后疼痛评分明显减轻(P<0.05),且治疗28天时试验组疼痛评分减轻较对照组明显(P<0.05)。治疗组治疗14、28天,TNF-α、IL-1β、IFN-γ、IL-2及IL-6较治疗前均明显下降,且治疗28天,TNF-α、IL-1β及IFN-γ较对照组下降更明显(P<0.05)。对照组治疗14、28天,IL-2、IL-6较治疗前下降(P<0.05);而TNF-α在治疗14天升高,但差异无统计学意义(P>0.05)。在治疗28天时,较治疗前下降,差异有统计学意义(P<0.05)。结论疮灵液外用可抑制局部的炎症,缩小PCM的局部肿块。Objective To observe the effects of external use of Chuangling Liquid(CLL)for reducing the local mass of plasma cell mastitis(PCM).Methods Forty PCM patients in need of dressing change were enrolled and assigned to the test group and the conrol group by random table digit,20 in each group.After incision and drainage of abscess or self-ulceration of abscess,patients in the test group received flushing with 30 mL PCM and drainage with CLL gauze strips.Patients in the control group were flushed with normal saline and with normal saline gauze strips.The dressing was changed twice per week for 4 successive weeks.The size of mass and pain score of patients were observed in all patients before and after treatment(before the 1 st changing),during dressing(on the 14 th day),after treatment(on the 28 th day).Local tissues were taken and examined by ELISA.Local inflammatory markers including IL-1β,IL-2,IL-6,IFN-γ,and TNF-αwere measured.Results Compared with before treatment in the same group,the mass was significantly reduced in the two groups(P<0.05).The reduction was more obviously in the test group than in the control group(P<0.05).The post-treatment pain score was obviously attenuated in the two groups(P<0.05).The post-treatment pain score was obviously lessened in the test group than in the control group(P<0.05).The levels of TNF-α,IL-1β,IFN-γ,IL-2,and IL-6 decreased in the test group at day 14 and 28,significantly lower than those before treatment(P<0.05).Besides,the levels of TNF-α,IL-1β,and IFN-γat day 28 decreased more significantly in the test group than in the control group(P<0.05).There were statistical differences in IL-2 and IL-6 levels in the control group between before treatment and 14 th/28 th day of treatment(P<0.05).TNF-αlevel increased on day 14 in the test group,but with no statistical difference(P>0.05).It decreased at day 28,as compared with before treatment(P<0.05).Conclusion External use of CLL inhibited local inflammation and reduced local mass of PCM patients.
