摘  要:目的探讨三水白虎汤(SSBH)对类风湿关节炎(RA)人血清肿瘤坏死因子(TNF-α)转基因(Tgtc)小鼠模型的疗效,初步探讨其抑制Tgtc小鼠骨质破坏的作用机制。方法 24只Tgtc小鼠按随机数字表法分为生理盐水(NS)组、SSBH组和来氟米特(LEF)组,每组8只。记录各组在给药期间的关节炎指数(AI),药物连续灌胃60天后进行双踝关节micro CT扫描及血清肿瘤坏死因子(TNF-α)、踝关节组织病理、免疫组化等检测。结果 SSBH、LEF组AI升高幅度、血清及踝关节滑膜组织TNF-α水平均较NS组低(P<0.05);SSBH、LEF组脾脏指数、踝关节骨质较NS组差异无统计学意义(P>0.05),但LEF组雌性鼠的骨质情况较其余两组的雌性鼠的骨质情况好(P<0.05);踝关节病理提示SSBH、LEF组的软骨层损伤深度、软骨表面损伤范围、软骨处炎症细胞浸润程度较NS组的低。结论 SSBH对Tgtc小鼠慢性关节炎有良好的抑制作用;并通过抑制炎症因子TNF-α的释放,一定程度上减缓Tgtc小鼠关节骨质破坏的进展。Objective To observe the effect of Sanshui Baihu Decoction(SSBHD) on human TNF alpha transgenic(Tgtc) mice with rheumatoid arthritis(RA),and to primarily explore its mechanism of inhibiting bone destruction. Methods Totally 24 Tgtc mice were divided into normal saline(NS), SSBH, and leflunomide(LEF) groups by random digit table, 8 in each group. The arthritis index(AI) of each group was recorded during administration. micro-CT was performed in two ankle joints;serum tumor necrosis factor alpha(TNF-α), ankle joint histopathology and immunohistochemistry were also evaluated after 60 days of continuous intragastric administration. Results Increased AI, TNF-α levels in serum and synovial tissue of ankle joint in SSBH and LEF groups were lower than those in NS group after intervention(P<0.05). There was no significant difference in spleen index and bony condition of ankle joint between SSBH/LEF group and NS group(P>0.05). But the bone condition of female mice in the LEF group was better than that of the other two groups(P<0.05). Ankle joint pathology showed that the depth of cartilage layer injury, the extent of cartilage surface injury, and the degree of inflammatory cell infiltration at cartilage were lower in SSBH and LEF groups than in NS group. Conclusion SSBH had favorable inhibitory effect on chronic arthritis in Tgtc mice, and slowed down the progress of bone destruction in Tgtc mice tosome extent possibly by inhibiting the release of inflammatory factor TNF-αlpha.
