楔形切除与改良全埋入式垂直褥式缝合在二次剖宫产腹壁切口中的应用

摘　　要：目的探讨楔形切除与改良全埋入式垂直褥式缝合在二次剖宫产腹壁切口中的应用。方法前瞻性自身对照研究,术前测量原瘢痕宽度和VSS评分,术后3、6、12个月再次测量原瘢痕宽度和VSS评分,记录瘢痕增生情况。结果术后12个月瘢痕宽度(0.52±0.26cm)、VSS评分(2.75±1.07),与术前的瘢痕宽度(1.01±0.35cm)、VSS评分(5.50±2.76)比较,差异有统计学意义(P=0.000)。结论楔形切除腹壁瘢痕及脂肪组织后采用改良的全埋入式垂直褥式缝合脂肪组织,显著减轻术后切口瘢痕过度增生及瘢痕变宽,切口瘢痕纤细,值得推广。Objective To investigate the application of wedge-shaped resection combined with modified embedded type vertical mattress suture in the abdominal incision for secondary cesarean section.Methods This is a self-controlled prospective study.The scar width and vancouver scar scale(VSS)score were measured before and at 3,6,and 12 months after surgery to evaluate the scar hyperplasia.Results The scar width(0.52±0.26cm vs 1.01±0.35cm)and VSS score(2.75±1.07 vs 5.50±2.76)at 12 months after surgery were significantly different from those before surgery(P=0.000).Conclusion Wedge-shaped resection of abdominal wall scars and adipose tissue,followed by adipose tissue sutured with modified embedded type vertical mattress method can significantly mitigate the scar widening and hyperplasia of postoperative incision.Therefore,this method is worthy of spreading to application for improved esthetic effects of the scars.
