下肢闭合穿钉辅助外固定支架治疗胫骨干骨折的临床疗效

摘　　要：目的探讨下肢闭合穿钉辅助外固定支架治疗胫骨干骨折中的临床疗效。方法根据手术方法不同将136例胫骨干骨折患者分为穿钉组和经皮组,每组68例。穿钉组采用下肢闭合穿钉辅助外固定支架治疗,经皮组采用经皮微创钢板内固定治疗。结果手术时间、术中出血量和骨折愈合时间穿钉组均短(少)于经皮组,差异均有统计学意义(P<0.05)。并发症发生率穿钉组低于经皮组,差异有统计学意义(P<0.05)。患者均获得12个月随访。术后9个月,根据Johner-Wruhs评分标准评价疗效:穿钉组优良率为97.1%,经皮组为83.8%,两组优良率比较差异有统计学意义(P<0.05)。结论胫骨干骨折患者应用下肢闭合穿钉辅助外固定支架治疗手术时间短,术中出血量少,并发症发生率低,治疗优良率高。Objective To investigate the clinical effect of the closed cross-pin assisted external fixation system in the treatment of tibial shaft fractures.Methods The 136 cases with tibial shaft fracture were selected,according to the operation method,were divided into the cross-pin group and percutaneous group,each of 68 cases.In the cross-pin group,the lower extremity was performed with closed crossing pin fixation assisted with external fixator system;the percutaneous group was treated with percutaneous minimally invasive plate internal fixation.Results The operation time,intraoperative bleeding volume and healing time of the cross-pin group were lower than those of the percutaneous group.There was statistically significant difference between the two groups(P<0.05).The incidence rate of complications in the cross-pin group was lower than that in the percutaneous group.There was statistically significant difference between the two groups(P<0.05).All patients were followed up for 12 months.At postoperative 9 months,according to the Johner-Wruhs score evaluation results,the excellent good rate in the cross-pin group of 97.1%was higher than 83.8%in the percutaneous group,there was statistical difference between the two groups(P<0.05).Conclusions Lower extremity closed cross-pin treatment assisted external fixation system in the treatment of tibial shaft fractures have the advantages of short operative time,less bleeding volume,low complication incidence rate and high excellent-god rate of therapy.
