鼻内窥镜下咽鼓管置管术与耳内窥镜下鼓膜切开置管术治疗慢性分泌性中耳炎患者的对比研究

摘　　要：目的分析比较鼻内窥镜下咽鼓管置管术与耳内窥镜下鼓膜切开置管术治疗慢性分泌性中耳炎患者的临床效果。方法选取2015年8月至2017年8月我院慢性分泌性中耳炎患者105例,按治疗术式不同分为咽鼓管置管组(n=53)及鼓膜切开置管组(n=52)。咽鼓管置管组采取鼻内窥镜下咽鼓管置管术,鼓膜切开置管组采取耳内窥镜下鼓膜切开置管术。术后随访6个月比较两组临床效果及并发症发生情况。结果咽鼓管置管组总有效率92.45%较鼓膜切开置管组76.92%高,差异有统计学意义(P<0.05);咽鼓管置管组术后并发症发生率5.66%较鼓膜切开置管组19.23%低,差异有统计学意义(P<0.05)。结论与耳内窥镜下鼓膜切开置管术相比,对慢性分泌性中耳炎患者采取鼻内窥镜下咽鼓管置管术治疗可提升临床效果,减少并发症发生。Objective To analyze and compare the clinical effects of eustachian catheterization under nasal endoscope and tympanotomy under the ear endoscope in the treatment of patients with chronic otitis media with effusion.Methods A total of 105 patients with chronic otitis media with effusion in our hospital from August 2015 to August 2017 were enrolled.The patients were divided into eustachian tube placement group(n=53)and tympanic membrane incision group(n=52).The eustachian tube placement group underwent endoscopic eustachian tube placement,and the tympanic membrane incision was performed under the ear endoscope.The clinical effects and complications of the two groups were compared after 6 months of follow-up.Results After follow-up,the total effective rate of the eustachian tube placement group was 92.45% higher than that of the tympanic membrane incision group(76.92%),the difference was statistically significant(P <0.05).The postoperative complication rate of the eustachian tube placement group 5.66% was lower than 19.23% of the tympanic membrane incision,and the difference was statistically significant(P<0.05).Conclusion Compared with endoscopic tympanotomy,the use of endoscopic eustachian tube placement in patients with chronic otitis media can improve clinical outcomes and reduce complications.
