银屑病关节炎患者的骨代谢水平及其临床意义

摘　　要：目的探讨银屑病关节炎患者的骨代谢水平及其临床意义。方法采用电化学发光免疫分析法检测30例银屑病关节炎患者(PA组)、30例寻常型银屑病患者(PV组)和30例健康体检者(C组)血清骨钙素(OC)、Ⅰ型原胶原N端前肽(PINP)和Ⅰ型胶原C端交联肽(CTX)的水平。分析骨代谢指标OC、PINP和CTX分别与患者年龄、病程、BMI、CRP和ESR等因素的相关性。结果PA组患者年龄(42.8±6.3)岁,关节炎病程27.0(12.0,204.0)个月,BMI(23.3±3.0)kg/m^2。与C组比较,PA组和PV组血清OC、PINP和CTX水平升高(P<0.05)。PA组CTX与银屑病病程、BMI呈负相关(r=-0.545、-0.486,P<0.05),与CRP、ESR呈正相关(r=0.529、0.515,P<0.05);PV组OC与CRP呈负相关(r=-0.672,P<0.05)。结论银屑病关节炎患者存在不同程度的骨代谢异常,且骨代谢水平与银屑病病程、BMI、CRP和ESR相关。炎症可能是银屑病关节炎患者骨代谢异常的重要因素。Objective To investigate the bone metabolism level and its clinical significance in the patients with psoriatic arthritis.Methods Serum levels of osteocalcin(OC),N-terminal propeptide of typeⅠprocollagen(PINP)and C-terminal cross-linking telopeptide of typeⅠcollagen(CTX)were detected by electrochemiluminescence immunoassay in 30 patients with psoriatic arthritis(group PA),30 patients with psoriasis vulgaris(group PV)and 30 healthy people(group C).The correlations of OC,PINP and CTX with the age,course of disease,body mass index(BMI),C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)were analyzed.Results The patients in group PA were(42.8±6.3)years old,the course of arthritis was 27.0(12.0,204.0)months,and BMI was(23.3±3.0)kg/m^2.Compared with group C,serum levels of OC,PINP and CTX were increased in groups of PA and PV(P<0.05).CTX was negatively correlated with the course of psoriasis and BMI(r=-0.545,-0.486,P<0.05),and positively correlated with CRP and ESR in group PA(r=0.529,0.515,P<0.05).OC was negatively correlated with CRP in group PV(r=-0.672,P<0.05).Conclusion There is an abnormal expression of bone metabolism level in the patients with psoriatic arthritis,which is closely related to the course of psoriasis,BMI,CRP and ESR.Inflammation may be an important factor for bone metabolism abnormality in the patients with psoriatic arthritis.
