摘　　要：目的探讨血清甘胆酸(CG)、总胆汁酸(TBA)联合检测对于妊娠期肝内胆汁淤积症(ICP)孕妇病情及围产儿不良结局的预测价值。方法选取某院2017年1月1日-2018年5月31日产科确诊的ICP孕妇114例为ICP组、同期正常妊娠孕妇100例为对照组,检测2组孕28周~孕36周的血清CG与TBA水平,根据围产儿结局对ICP组进行分层分析;绘制ROC曲线分析血清CG、TBA水平预测围产儿不良结局的价值。结果ICP组孕妇血清CG、TBA水平显著高于对照组;ICP组围产儿不良结局28.07%显著高于对照组的8.00%;ICP组围产儿不良结局孕妇的血清CG、TBA水平显著的高于围产儿正常结局组孕妇;绘制ROC曲线,血清CG、TBA水平联合应用预测ICP孕妇围产儿不良结局的灵敏度为89.64%、特异度为82.56%、ROC曲线下面积AUC值为0.858。结论血清CG与TBA水平联合检测对于ICP的诊断及ICP产妇围产儿不良结局预测具有一定的临床价值。Objective To explore the predictive value of combined detection of serum cholyglycine acid(CG)and total bile acid(TBA)for the condition of pregnant women with intrahepatic cholestasis of pregnancy(ICP)and adverse perinatal outcomes.Methods From January 1 st,2017 to May 31 st,2018,the levels of serum CG and TBA were measured in 114 ICP pregnant women(ICP group)and 100 normal pregnant women(control group)during the same period.The levels of serum CG and TBA were stratified according to perinatal outcomes.The ROC curve was drawn to analyze the value of serum CG and TBA levels in predicting adverse perinatal outcomes.Results The levels of serum CG and TBA in ICP group were significantly higher than those in control group;the adverse perinatal outcomes in ICP group were 28.07% higher than those in control group;the levels of serum CG and TBA in ICP group were significantly higher than those in normal perinatal outcomes group;the ROC curve was drawn,and the levels of serum CG and TBA were combined to predict the adverse perinatal outcomes in ICP group.The sensitivity was 89.64%,the specificity was 82.56%,and the AUC value under the ROC curve was 0.858.Conclusions The combined detection of serum CG and TBA levels had certain clinical value for the diagnosis of ICP and the prediction of adverse perinatal outcomes of ICP.
