摘 要:目的探讨大骨瓣减压手术治疗颅脑损伤患者中应用不同麻醉方法的临床效果,并分析两组患者的应激反应。方法选择我院2016年3月至2018年3月期间所收治的80例颅脑损伤患者作为本次研究对象,按照麻醉方法的不同将其分为研究组和对照组,每组各40例,对照组患者采取静吸复合麻醉,研究组患者则应用全屏静脉麻醉,比较两组患者的麻醉效果与患者的应激反应表现。结果研究组自主呼吸恢复时间(5.83±1.24)min、拔管时间、苏醒时间(5.87±1.01)min、言语应答时间与定向力恢复时间均明显少于对照组,组间差异显著;两组手术前应激反应指标比较无统计学意义(P>0.05),麻醉维持前与手术结束时两组应激反应指标均显著高于手术前,手术结束时,研究组的各项指标显著优于对照组(P<0.05),组间比较,存在明显统计学意义。结论颅脑损伤患者应用大骨瓣减压手术治疗过程中应用全屏静脉麻醉的效果明显,能够有效缩短患者的手术后苏醒时间、自主呼吸恢复时间以及拔管时间等,且对抑制手术中出现的应激反应有明显优势,值得临床进一步应用探索。Objective To investigate the clinical effects of different anesthesia methods in patients with craniocerebral injury treated with large bone flap decompression,and to analyze the stress response of the two groups.Methods Eighty patients with road brain injury admitted to our hospital from March 2016 to March 2018 were selected as the study subjects.According to the different anesthesia methods,they were divided into study group and control group,40 cases in each group.Patients in the control group underwent combined intravenous anesthesia,and patients in the study group underwent full-screen intravenous anesthesia to compare the anesthetic effects of the two groups with the patient's stress response.Results The study group's spontaneous breathing recovery time(5.83±1.24)min,extubation time,recovery time(5.87±1.01)min,verbal response time and orientation recovery time were significantly less than the control group,and the difference between the groups was significant;The stress response index was not statistically significant(P>0.05).The stress response indexes of the two groups before and after the anesthesia maintenance were significantly higher than those before the operation.At the end of the operation,the indexes of the study group were significantly better than the control group(P<0.05),there was statistically signifi cant difference between groups.Conclusion The effect of full-screen intravenous anesthesia in patients with craniocerebral injury treated with large bone flap decompression is obvious,which can effectively shorten the post-operative recovery time,spontaneous breathing recovery time and extubation time,etc.The stress response that occurs in the treatment has obvious advantages and is worthy of further clinical application.
