摘  要:目的观察尿激酶联合抗结核药胸腔内注入治疗结核性包裹性胸腔积液的效果。方法我院2017年7月至2018年7月收治的120例结核性包裹性胸腔积液患者为本次研究对象,按照是否给予尿激酶联合抗结核药胸腔内注入治疗将所有患者分为对照组(60例:常规胸腔穿刺引流治疗)与实验组(60例:尿激酶联合抗结核药胸腔内注入治疗),比较两组患者预后。结果实验组患者治疗7 d后肺活量(SVC)、最大通气量(MVV)、第1秒用力肺活量(FEV1)等肺功能检测数值和凝血酶时间、凝血酶原时间以及部分凝血酶原时间等凝血指征均明显优于对照组,差异具有统计学意义(P<0.05)。实验组治疗14 d后总有效率为98.33%,明显高于对照组,差异具有统计学意义(P<0.05)。实验组患者胸膜增厚、胸水吸收时间以及平均住院时间等相关指标与对照组相比,差异具有统计学意义(P<0.05)。结论结核性包裹性胸腔积液患者尿激酶联合抗结核药胸腔内注入治疗明显优于常规胸腔穿刺引流治疗。Objective To observe the effect of intrathoracic injection of urokinase combined with anti-tuberculosis drugs in the treatment of tuberculous encapsulated pleural effusion.Methods 120 patients with tuberculous encapsulated pleural effusion admitted to our hospital from July 2017 to July 1818 were enrolled in this study.All patients were divided into controls according to whether or not urokinase combined with anti-tuberculosis drugs were administered intrathoracic injection.The group(60 cases:conventional thoracic puncture drainage treatment)and the experimental group(60 cases:urokinase combined with anti-tuberculosis drug intrathoracic injection),compared the prognosis of the two groups.Results The blood pressure(SVC),maximum ventilation(MVV),fi rst second forced vital capacity(FEV1)and other lung function tests and thrombin time,prothrombin time and partial prothrombin time were measured after 7 days of treatment.The indications were signifi cantly better than the control group,and the difference was statistically signifi cant(P<0.05).After 14 days of treatment,the total effective rate was 98.33%,which was signifi cantly higher than that of the control group,and the difference was statistically significant(P<0.05).The pleural thickening,pleural effusion time and average length of hospital stay in the experimental group were signifi cantly different from those in the control group(P<0.05).Conclusion Intrathoracic injection of urokinase combined with antituberculosis in patients with tuberculous encapsulated pleural effusion is superior to conventional thoracic drainage.
