宫腔灌注粒细胞刺激因子对反复种植失败患者妊娠结局的作用

摘　　要：目的探讨粒细胞刺激因子(granulocyte colony-stimulating factor,G-CSF)宫腔内灌注治疗对反复种植失败(recurrent implantation failure,RIF)患者胚胎种植率和临床妊娠率的影响。方法将纳入研究的59例拟行冷冻胚胎移植的RIF患者随机分为对照组(29例)和治疗组(30例)。治疗组行宫腔灌注G-CSF治疗,对照组口服强的松5mg治疗。结果治疗组与对照组患者的一般情况(年龄、BMI、不孕年限、血清卵泡刺激素、黄体生成素、雌二醇、既往取卵周期数、移植周期数、移植胚胎数及单周期移植优质胚胎数)均差异无统计学意义(P>0.05)。治疗组胚胎种植率(23.8%vs 11.0%)以及临床妊娠率(53.3%vs 17.2%)均高于对照组(P<0.05)。结论G-CSF宫腔内灌注可提高RIF患者胚胎种植率和临床妊娠率,对提高RIF患者妊娠率具有一定意义。Objective To investigate the effect of intrauterine infusion of granulocyte colony-stimulating factor(G-CSF)on embryo implantation rate and clinical pregnancy rate in patients with recurrent implantation failure(RIF).Methods A total of 59 RIF patients,who were enrolled in frozen embryo transfer,were randomly divided into control group(n=29)and treatment group(n=30).The treatment group received intrauterine infusion of G-CSF and the control group received oral prednisone(5 mg).Results There was no significant difference between the treatment group and the control group in age,BMI,infertility duration,serum follicle stimulating hormone,luteinizing hormone,estradiol,egg retrieval cycles,transplantation cycles,transplanted embryos,and high-quality embryos per single cycle(P>0.05).The embryo implantation rate(23.8%vs.11.0%)and the clinical pregnancy rate(53.3%vs.17.2%)were significantly higher in the treatment group than in the control group(P<0.05).Conclusion G-CSF intrauterine infusion can improve the embryo implantation rate and clinical pregnancy rate of RIF patients,and it has certain significance for improving pregnancy rate in RIF patients.
