完全无管化经皮肾镜取石术的安全性及有效性分析

摘　　要：目的分析完全无管化经皮肾镜碎石术的手术适应症,以及安全性和有效性。方法回顾性分析本院2017年3月至2019年5月收治的肾结石患者93例,其中完全无管化PCNL(无管化组)46例,标准PCNL(标准组)47例。比较两组的手术时间、住院时间、术后并发症发生率、结石清除率以及手术前后血红蛋白、血肌酐变化。结果无管化组的住院时间显著短于标准组,并发症发生率显著低于标准组(P<0.05)。两组的手术时间、结石清除率以及血红蛋白、血肌酐变化无明显差异(P>0.05)。结论对于小于3 cm的肾结石、术中出血少、无需二次取石的患者,完全无管化PCNL具有手术并发症少、住院时间短等优点。Objective To analyze the indications,safety and effectiveness of total tubeless percutaneous nephrolithotripsy(PCNL).Methods 93 patients with renal calculi in our hospital from March 2017 to May 2019 were analyzed retrospectively,including 46 cases of total tubeless PCNL(tubeless group)and 47 cases of standard PCNL(standard group).The operation time,hospitalization time,incidence of postoperative complications,stone clearance rate,and the changes of hemoglobin and creatinine before and after surgery were compared between the two groups.Results The hospitalization time in the tubeless group was significantly shorter than that in the standard group,and the incidence of complications was significantly lower than that in the standard group(P<0.05).No significant difference was found in operation time,stone clearance rate,and changes of hemoglobin and creatinine between the two groups(P>0.05).Conclusions For patients with renal calculi less than 3 cm,less intraoperative bleeding and unnecessary second stone removal,total tubeless PCNL has the advantages of fewer operative complications and shorter hospitalization time.
