腕关节镜技术结合切开复位内固定手术治疗桡骨远端关节内骨折的早期效果观察

摘　　要：目的探讨腕关节镜技术结合切开复位内固定手术治疗桡骨远端关节内骨折患者的早期效果。方法82例桡骨远端关节内骨折患者随机分为两组各41例。观察组给予腕关节镜技术结合切开复位内固定手术治疗,对照组给予单纯切开复位内固定手术治疗。比较两组的手术指标,以及术后1年的腕关节活动范围、Gartland/Werley腕关节评分及腕关节功能。结果观察组的手术时间显著长于对照组(P<0.05),两组的术中出血量无统计学差异(P>0.05)。术后1年,观察组的腕关节活动度(屈曲、背伸、尺偏、桡偏),前臂旋前、旋后活动范围,以及腕关节功能优良率均显著高于对照组(P<0.05);观察组的Gartland/Werley腕关节评分显著低于对照组(P<0.05)。结论腕关节镜技术结合切开复位内固定手术治疗桡骨远端关节内骨折具有显著的临床疗效,可改善患者的腕关节功能,促进关节面恢复。Objective To explore the early effect of wrist arthroscopy technique combined with open reduction and internal fixation in the treatment of patients with intra-articular fracture of distal radius.Methods 82 patients with intra-articular fracture of distal radius were randomly divided into two groups.The observation group was treated with wrist arthroscopy technique combined with open reduction and internal fixation surgery,while the control group was only treated with open reduction and internal fixation surgery.The surgical indicators,and the activity range of wrist joint,Gartland/Werley wrist joint score and wrist joint function 1 year after surgery were compared between two groups.Results The surgery time of the observation group was significantly longer than that of the control group(P<0.05);No significant difference was found in intraoperative blood loss between two groups(P>0.05).1 year after surgery,the range of motion of wrist joint(flexion,dorsiflexion,ulnar deviation,radial deviation),the activity range of forearm pronation and supination,and the excellent and good rate of wrist joint function of the observation group were significantly higher than those of the control group(P<0.05);The Gartland/werley wrist joint score of the observation group was significantly lower than that of the control group(P<0.05).Conclusions Wrist arthroscopy technique combined with open reduction and internal fixation surgery in the treatment of intra-articular fracture of distal radius has significant clinical efficacy,which can improve the wrist joint function and promote the recovery of articular surface of patients.
