注射用环磷腺苷葡胺致18例新的或严重的不良反应临床分析

摘　　要：目的探讨注射用环磷腺苷葡胺新的或严重的不良反应(ADR),为临床合理用药提高参考。方法采用回顾性分析,对我院2015年1月1日至2018年2月28日我院上报至国家药品不良反应监测中心静脉滴注注射用环磷腺苷葡胺致18例新的或严重的ADR报告进行分析整理,包括患者一般情况、原患疾病、用法用量、ADR发生时间、联合用药情况、累及系统/器官、临床表现、治疗及转归、关联性及严重程度评价等项目进行统计分析。结果18例ADR报告中,男性多于女性,60岁以上患者居多;61.1%ADR发生在第1次静脉滴注时/后;累及系统/器官最常见的是皮肤和心血管系统,表现为瘙痒、皮疹、面部潮红、胸闷等。研究发现2例致严重ADR事件,1例患者突发意识丧失,1例患者出现胸闷、憋喘、呼吸困难和寒战。结论医护人员应加强监护静滴注射用环磷腺苷葡胺后患者的反应,减少药品导致的严重伤害。Objective To investigate the new or severe adverse reaction(ADR)of meglumine adenosine cyclophosphate for Injection,so as to improve the reference for rational clinical use.Methods A retrospective analysis of our hospital on January 1,2015-February 28,2018,our hospital reported to the national adverse drug reaction monitoring center,18 cases of new or severe ADR induced by meglumine adenosine cyclophosphate for Injection were analyzed,including patient general condition,the original disease,usage and dosage,ADR occurrence time,combination,involving system/organs,clinical manifestations,treatment and outcome,relevance and severity evaluation project were analyzed.Results In 18 cases of ADR reports,more males than females,with the majority of patients were over 60 years old.61.1%ADR occurred at/after the first intravenous drip.The most commonly involved systems/organs were the skin and cardiovascular systems,manifested as pruritus,rashes,facial flushing,chest tightness,etc.The study found that 2 cases caused severe ADR events,1 patient suddenly lost consciousness,and 1 patient had chest tightness,wheezing,dyspnea and chills.Conclusion Medical staff should strengthen the monitoring of patients'response after the use of meglumine adenosine cyclophosphate for Injection to reduce the serious injury caused by drugs.
