磁共振尿路成像在上尿路梗阻性疾病诊断中的价值

摘　　要：目的分析磁共振尿路成像(MRU)在上尿路梗阻性疾病诊断中的价值。方法选取本院2016年3月至2018年9月收治的67例上尿路梗阻性疾病患者,全部患者均行MRU、B超、静脉尿路造影(IVU)和多排螺旋CT(MDCT)检查,以临床综合诊断结果为参考依据,分析上述四种影像学检查方法诊断尿路梗阻性疾病诊断的结果。结果67例尿路梗阻性疾病患者输尿管结石占比(29.85%),肾结石占比(17.91%),泌尿系畸形占比(17.91%),输尿管癌占比(14.93%),输尿管炎性狭窄占比(8.96%);肾盂癌例占比(5.97%);输尿管结核占比(4.48%);MRU诊断尿路梗阻性疾病的准确率(94.03%)高于B超(53.73%)和IVU(68.66%),差异有统计学意义(P<0.05);MDCT诊断尿路梗阻性疾病的准确率为(86.57%),与MRU诊断准确率比较,差异无统计学意义(P>0.05)。结论MRU诊断上尿路梗阻性疾病准确率较高,具有重要的临床应用价值。Objective To analyze the value of magnetic resonance urography(MRU)in the diagnosis of upper urinary tract obstruction.Methods 67 patients with urinary tract obstructive disease admitted to our hospital from March 2016 to September 2018 were enrolled.All patients underwent MRU,B-ultrasound,intravenous urography(IVU)and multi-slice spiral CT(MDCT).The results of the diagnosis of urinary tract obstructive diseases were analyzed by the above four imaging examination methods based on the clinical comprehensive diagnosis results.Results Of the 67 patients with urinary tract obstructive disease,the proportion of ureteral stones(29.85%),the proportion of kidney stones(17.91%),the proportion of urinary malformations(17.91%),the proportion of ureteral cancer(14.93%),ureteritis The proportion of stenosis(8.96%);the proportion of renal pelvis cancer(5.97%);the proportion of ureteral tuberculosis(4.48%);the accuracy of MRU diagnosis of urinary tract obstruction disease(94.03%)is higher than B-ultrasound(53.73%)and IVU(68.66%),the difference was statistically significant(P<0.05);the accuracy rate of MDCT in the diagnosis of urinary tract obstructive disease was(86.57%).Compared with the diagnostic accuracy of MRU,the difference was not statistically significant(P>0.05).Conclusion MRU has a high accuracy in the diagnosis of urinary tract obstructive diseases and has important clinical application value.
