自体骨移植与同种异体骨治疗胫骨平台塌陷骨折的效果比较

摘　　要：目的:比较自体骨移植与同种异体骨治疗胫骨平台骨折的效果。方法:选择2014年6月—2018年6月手术治疗的38例成人胫骨平台骨折患者。根据随机原则分成两组:自体骨移植组18例,同种异体骨组20例。对术中失血量、手术时间、骨折愈合时间、骨折愈合率、随访结束时膝关节HISS评分、感染发生率、术后再塌陷率等指标进行比较。结果:自体骨移植组术中失血量、手术时间明显多于同种异体骨组,且差异有统计学意义(P<0.05)。随访结束时,所有骨折均愈合,术后再塌陷率发生率自体骨移植组高于同种异体骨组(17%vs.5%),但差异无统计学意义(P>0.05)。两组骨折愈合时间、随访结束时膝关节HISS评分差异无统计学意义(P>0.05)。结论:对于胫骨平台骨折的治疗,同种异体骨可以获得与自体骨移植一样的骨折愈合率及疗效,但是可以明显减少术中失血量及手术时间,同种异体骨可能是更好的填充支撑物。Objective To compare the clinic effects of autogenous bone graft with allograft in treatment for depressed tibial plateau fractures.Methods From Jun 2014 to Jun 2018,38 cases of depressed tibial plateau fractures were divided into two groups which autogenous bone graft group were 18 cases and allograft group were 20 cases.These indexes such as intraoperative blood loss,operation time,fracture union time,fracture union,the HISS score at the end of follow-up,the incidence of infection and recurrent depression were collected to compare between the two groups.Results Compared with the allograft treatment,autogenous bone graft were more significantly intraoperative blood loss and operation time,and the difference had statistical significance(P<0.05).At the end of followup,all fractures union but the incidence of recurrent depression in autogenous bone graft group higher than allograft group(17%vs.5%),but the difference had no statistical significance(P>0.05).There were no statistically significant differences regarding fracture union time,the HISS score at the end of follow-up between the 2 groups(P>0.05).Conclusion For the treatment of tibial plateau fracture,allograft can achieve the same fracture healing rate and efficacy as autogenous bone graft,but it can significantly reduce intraoperative blood loss and operative time,allograft may be a better choice for filler.
