摘 要:目的探讨子痫前期孕妇胎盘组织中miR-34a和沉默信息调控子1(SIRT1)表达情况,并探讨其在子痫前期疾病中的作用。方法选取2016年9月至2018年9月住院择日行剖宫产的子痫前期孕妇63例,其中子痫前期轻度组29例和子痫前期重度组34例,另选取同期因胎位不正等原因经剖宫产分娩的正常妊娠孕妇31例作为对照组。采用qRT-PCR法检测三组胎盘组织中miR-34a与SIRT1 mRNA相对表达量,免疫组化法检测胎盘组织中SIRT1蛋白表达。应用Pearson法对miR-34a与SIRT1 mRNA水平进行相关性分析。结果与对照组比较,子痫前期轻度组、重度组收缩压、舒张压、平均脉动压、24 h蛋白尿均升高(P<0.05),且子痫前期重度组孕妇收缩压、舒张压、平均脉动压、24 h蛋白尿水平高于轻度组(P<0.05);与对照组孕妇相比,子痫前期轻度组、重度组胎盘组织miR-34a表达显著升高(P<0.05),SIRT1 mRNA水平及蛋白阳性表达率显著降低(P<0.05);与子痫前期轻度组孕妇比较,重度组胎盘组织miR-34a水平显著升高(P<0.05),SIRT1 mRNA水平及蛋白阳性表达率显著降低(P<0.05);Pearson相关性显示,子痫前期孕妇胎盘组织中miR-34a与SIRT1 mRNA表达水平呈负相关(r=-0.874,P<0.05)。结论子痫前期孕妇胎盘组织中miR-34a高表达,SIRT1低表达,miR-34a可能通过下调SIRT1表达参与子痫前期疾病发生。Objective To investigate the expressions of miR-34a and silent information regulator-1(SIRT1)in placenta tissues of pregnant women with preeclampsia,and to explore their role in preeclampsia.Methods A total 63 pregnant women with preeclampsia who underwent cesarean section in our hospital from September 2016 to September 2018 were enrolled in the study,of whom,there were 29 cases of mild preeclampsia(mild preeclampsia group)and 34 cases of severe preeclampsia(severe preeclampsia group),moreover,the other 31 normal pregnant women who underwent cesarean section due to improper fetal position at the same period were enrolled as control group.The relative expression levels of miR-34a and SIRT1 mRNA in placenta tissues were detected by qRT-PCR,the expression levels of SIRT1 protein in placenta were detected by immunohistochemistry.Pearson method was used to analyze the correlation between the expression of SIRT1 and the expression of miR-34a.Results As compared with those in control group,the systolic pressure,diastolic pressure,mean pulsatile pressure,and 24-hour proteinuria in mild preeclampsia group and severe preeclampsia group were significantly increased(P<0.05),moreover,the levels of systolic pressure,diastolic pressure,mean pulsatile pressure,and 24-hour proteinuria in severe preeclampsia group were significantly higher than those in mild preeclampsia group(P<0.05).As compared with those in control group,the expression levels of miR-34a in in mild preeclampsia group and severe preeclampsia group were significantly increased(P<0.05),whereas,the SIRT1 levels and protein positive expression rate were significantly decreased(P<0.05).In addition the levels of miR-34a in severe preeclampsia group were significantly higher than those in mild preeclampsia group(P<0.05),but the SIRT1 levels and protein positive expression rate were significantly lower than those in mild preeclampsia group(P<0.05).Pearson correlation showed that the expression levels of SIRT1 were negatively correlated with the expression levels of miR-
