摘 要:目的通过超声生物显微镜(UBM)观察高度近视患者晶状体悬韧带长度,研究晶状体悬韧带的长度对高度近视白内障术后人工晶状体稳定性的影响。方法选择高度轴性近视眼组和正视眼组各30例,均接受透明角膜隧道小切口超声乳化白内障吸出折叠式人工晶状体植入术治疗,植入同一类型后房型人工晶状体(IOL),用UBM测量两组晶状体悬韧带长度、前房深度、IOL偏心量、IOL倾斜度。结果两组内12点位、3点位、6点位、9点位悬韧带长度比较差异无统计学意义(P>0.05),组间4个钟点位悬韧带长度比较,差异有统计学意义(P<0.05)。高度轴性近视眼组眼轴长度及平均悬韧带长度长于正视眼组(P<0.05)。经直线相关分析显示,眼轴长度与平均悬韧带长度呈正相关(r=0.897,P<0.05)。正视眼组及高度轴性近视眼组术后3个月前房深度较术后1个月上升,组内、组间差异有统计学意义(P<0.05)。正视眼组及高度轴性近视眼组术后1个月和术后3个月IOL偏心量、IOL倾斜度比较,组内差异无统计学意义(P>0.05),但组间差异有统计学意义(P<0.05)。结论晶状体悬韧带的长度随着眼轴长度的增加而相应增加,且悬韧带长度的增长可表现为IOL不稳定性增加。Objective To observe the effect of suspensory ligament length on the stability of intraocular lens after cataract surgery in patients with high myopia.Methods Thirty cases were enrolled in the high myopia group and emmetropia group,respectively.All patients were treated with transparent corneal tunnel small incision phacoemulsification cataract extraction,and the same type of posterior chamber intraocular lens(IOL)implantation were performed.UBM was used to measure the suspensory ligament length,anterior chamber depth(ACD),IOL bias and IOL inclination in the two groups.Results There were no statistically significant differences in the length of suspensory ligament at 12,3,6 and 9 o’clock in the two groups(P>0.05),but there were statistically significant differences in the length of suspensory ligament at 4 o’clock between the two groups(P<0.05).Axial length(AL)and mean suspensory ligament length in the high myopia group were longer than those in the emmetropia group(P P<0.05).Linear correlation analysis showed that AL was positively associated with mean suspensory ligament length(r=0.897,P<0.05).The depth of anterior chamber at 3 months after the operation in the two groups was larger than that at 1 month after the operation,and the differences within each group and between the two groups were statistically significant(P<0.05).Comparison of IOL eccentricity and IOL inclination at 1 month and 3 months after the operation showed no statistically significant intergroup difference(P>0.05),but there were statistically significant differences between the groups(P<0.05).Conclusions The length of suspensory ligament increased with longer axial length,and the increase in the length of suspensory ligament can increase the instability of IOL.
