摘 要:目的观察改良注射器负压在硅油限速取出术中的临床应用效果。方法选取2018年1月至2019年8月,来我科室住院行硅油取出术的患者22例(22只眼),随机抽取11例纳入改良组,11例纳入常规组。改良组用保留流速调节阀的一次性输液器,连接20 ml注射器,进行硅油的限速取出。观察并比较两组取油时间、速度、取油前后期的灌注液滴速比、气液交换次数、术后眼压和视力。结果两组患者在取油时间、速度、术后1 d和1周眼压、视力构成等比较,差异均无统计学意义(P>0.05);而滴速比、气液交换次数,差异有统计学意义(P<0.05)。结论改良注射器负压在硅油取出术中的应用,可以限制取油末期的抽吸速度,使整个取油过程更平稳,取油更彻底,减少了气液交换的次数,对眼内压的扰动更小,有利于手术安全。Objective To observe the clinical effect of negative pressure generated by the modified syringe in silicone oil removal(SOR)with limited speed.Methods Twenty-two patients(22 eyes)who underwent SOR from January 2018 to August 2019 in our hospital were enrolled.The patients were randomly assigned into the modification group(11 eyes)and the conventional group(11 eyes).A cut disposable infusion set with an adjuster retained was connected to a 20ml syringe to carry out the SOR with limited speed in the modification group.The time and speed of SOR,the ratio of infusion droplet velocity at early and late stage of the operation,the number of fluid-air exchange,the intraocular pressure(IOP)and visual acuity were observed and compared between the two groups.Results There were no statistically significant differences in the time and speed of SOR,IOP or visual acuity between the two groups(P>0.05).There were statistically significant differences in the droplet velocity rate and the number of fluid-air exchange between the two groups(P<0.05).Conclusions The clinical application of negative pressure generated by the modified syringe in SOR with limited speed can limit the suction speed at the end of the operation,making the whole process more stable and complete,and also can reduce the number of fluid-air exchange,resulting in less disturbance to IOP and better surgical safety.
