摘  要:目的分析数字乳腺断层融合X线成像(digital breast tomosynthesis,DBT)中平均腺体剂量(average glandular dose,AGD)与压迫厚度、乳腺密度的相关性。方法回顾性分析80例行双侧乳腺摄影的女性患者临床资料,分别用DBT与全视野数字化乳腺摄影(full-field digital mammography,FFDM)在同一乳腺、同一压迫厚度下进行摄影,分析所搜集的单侧FFDM乳腺头尾位图像,计算△AGD值(DBT-AGD值与FFDM-AGD值之差)。采用Pearson相关分析和多重线性回归分析DBT中AGD与压迫厚度、乳腺密度的关系。结果DBT-AGD值与压迫厚度呈现显著正相关(r=0.90,P<0.01),且与乳腺密度呈现一定相关性(r=0.27,P<0.05)。乳腺密度与压迫厚度均是DBT-AGD值的影响因素(b值分别为0.09、0.05,均P<0.05)。△AGD值与压迫厚度(r=-0.30,P<0.05)、乳腺密度(r=-0.64,P<0.01)呈现负相关关系,压迫厚度与乳腺密度均是△AGD的影响因素(b值分别为-0.03、-0.20,均P<0.05)。在压迫厚度为56~75 mm且乳腺密度为51%~75%时,及在压迫厚度为76~95 mm且乳腺密度为25%~50%时,DBT中AGD值与FFDM中AGD值比较,差异均无统计学意义(P>0.05)。在压迫厚度为56~75 mm且乳腺密度超过75%时,DBT中的AGD值较FFDM中的AGD值明显下降;在压迫厚度不超过95 mm且其它乳腺密度情况下,DBT中的AGD值较FFDM中的AGD值明显升高;差异均有统计学意义(P<0.05)。结论DBT中AGD与压迫厚度和乳腺密度密切相关,在乳腺腺体密度不变的条件下减少乳腺压迫厚度对降低AGD有积极作用。Objective To analyze correlation of average glandular dose(AGD)with compression thickness and breast density in digital breast tomosynthesis(DBT).Methods The clinical data of 80 female patients with bilateral mammography were analyzed retrospectively.DBT and full-field digital mammography(FFDM)were performed under the same breast and the same compression thickness.The head and tail images of unilateral FFDM were collected,and calculated△AGD(DBT-AGD minus FFDM-AGD).Pearson correlation analysis and multiple linear regression analysis were used to analyze the correlation of AGD with compression thickness and breast density in DBT.Results DBT-AGD was positively correlated with compression thickness(r=0.90,P<0.01)and breast density(r=0.27,P<0.05).Breast density and compression thickness were the influencing factors of DBT-AGD(b values were 0.09 and 0.05,P<0.05).△AGD was negatively correlated with compression thickness(r=-0.30,P<0.05)and breast density(r=-0.64,P<0.01).Compression thickness and breast density were the influencing factors of△AGD(b values were-0.03 and-0.20,P<0.05).There was no significant difference between the AGD value of DBT and that of FFDM when the compression thickness was 56-75 mm and the breast density was 51%-75%(P>0.05).There was no significant difference between the AGD value of DBT and that of FFDM when the compression thickness was 76-95 mm and the breast density was 25%-50%(P>0.05).AGD values in DBT were lower than those in FFDM when the compression thickness was 56-75 mm and breast density was more than 75%.AGD values in DBT were higher than those in FFDM when the compression thickness was not more than 95 mm and the breast density was less than 25%.The differences were statistically significant(P<0.05).Conclusions In DBT,AGD is closely related to both compression thickness and breast density,and the reduction of breast compression thickness has a positive effect on the reduction of AGD.
