内侧半月板损伤关节镜下治疗与髌内侧滑膜皱襞的相关性

摘　　要：目的探讨关节镜下治疗内侧半月板损伤与髌内侧滑膜皱襞(mediopatellar plica,MPP)的相关性。方法收集2016年1月-2018年1月在合肥市第三人民医院行关节镜治疗的18例内侧半月板损伤患者的临床资料。关节镜下行内侧半月板成形术或者缝合术,MPP在关节镜下行清除。依据关节镜下是否存在MPP,分成2组;关节镜下有无病理性MPP,分成2组,手术前后膝关节功能采用Lysholm评分,数据间比较采用协方差分析,两因素相关性分析采用Spearman分析。结果无MPP组术前Lysholm评分(38.36±3.12)分,术前有MPP组(30.43±1.75)分;术后无MPP组Lysholm评分(78.91±4.52)分,术后有MPP组(70.36±3.71)。术前A、B型MPP组Lysholm评分(38.37±3.23)分,术前C,D型MPP评分(30.78±1.10)分;术后A、B型MPP组Lysholm评分(79.75±2.90)分;术后C,D型MPP评分(70.27±2.81)分,经协方差分析结果均有统计学意义(均P<0.05),提示MPP和型别可能会影响手术疗效。Spearman相关分析,显示术后Lysholm评分与含有MPP者呈负相关(相关系数r=-0.792,P<0.05)。结论MPP影响膝关节内侧半月板损伤患者疗效,关节镜下需准确清除MPP,尤其是C、D型MPP。Objective To investigate the correlation between medial meniscus injury and mediopatellar plica(MPP)under arthroscopy.Methods Eighteen patients with medial meniscus injury who received arthroscopic treatment in our hospital from January 2016 to January 2018 were collected.Medial meniscus plasty or suture was performed,and MPP was removed under arthroscopy.According to the presence or absence of MPP under arthroscopy,the patients were divided into two groups.According to whether there is pathological MPP under arthroscopy,the patients were divided into two groups.Lysholm score was used for knee function before and after surgery.Covariance analysis was used to compare the data,and Spearman analysis was used to analyze the correlation between the two factors.Results Before surgery,the preoperative Lysholm score of the no MPP group(38.36±3.12)was higher than that of the MPP group(30.43±1.75),and after surgery,the postoperative Lysholm score of the no MPP group(78.91±4.52)was higher than that of the MPP group(70.36±3.71).Before surgery,the preoperative Lysholm score of MPP group A and B(38.37±3.23)was higher than that of C and D group(30.78±1.10),and after surgery,the postoperative Lysholm score of MPP group A and B(79.75±2.90)was higher than that of C and D group(70.27±2.81,all P<0.05).Spearman correlation analysis showed that the postoperative Lysholm score was negatively correlated with MPP content(correlation coefficient r=-0.792,P<0.05).Conclusion MPP affects the efficacy of patients with medial meniscus injury of the knee joint,and accurate removal of MPP,especially type C and D,is required under arthroscopy.
