摘　　要：目的总结我国大陆地区1990-2017年报道的挪威疥病例,探讨其流行特点、临床特征和诊疗情况。方法检索1990年1月-2017年12月中英文文献数据库中所有关于"挪威疥"的病例报道文献,统计分析所有患者的基本特征、病史、临床表现、实验室检查、治疗以及预后等相关信息。结果文献检索获得病例报道39篇,共43例;平均发病年龄(53.7±27.2)岁,男性25例,女性18例,病程数周到1年。多数患者(40例,93%)长期卧床或合并严重的基础疾病,临床表现为"角化性痂皮或鳞屑",最常见于手足部位,多伴剧烈瘙痒。所有患者均经病原学检查确诊,30例误诊,最常误诊为湿疹和红皮病。治疗主要以硫乳膏、克罗米通等杀虫制剂为主,大多预后良好(38例痊愈、3例显著好转)。结论挪威疥是一种病情严重的疥螨寄生性皮肤病,传染性大,临床误诊率高,对患者健康及其家庭影响甚大。提高认识,仔细问诊病史,熟悉皮损特点,及时开展病原学检查,有利于疾病的早期诊治和流行病学防控。Objective To investigate the epidemiological and clinical characteristics of Norwegian scabies in China mainland(1990-2017). Methods Search all the case reports of Norwegian scabies from the Chinese and English databases(from January 1990 to December 2017). The basic characteristics, medical history, clinical manifestations, laboratory tests, treatment and prognosis of reported cases were analyzed. Results Forty-three cases of Norwegian scabies were reported in 39 articles from the electrical database. There were 25 males and 18 females with a mean age of(53.7±27.2) years and the course ranged from several weeks to 1 year. Of them, 40 patients(93%) were long-term bedridden or those with severe underlying disease. The major clinical manifestations were keratinized crust or scales with severe itching, most commonly in the hands and feet. All patients were diagnosed by pathogenic examination, while 30 cases were initially misdiagnosed as eczema and erythroderma. The sulfur cream, crotamiton, or other insecticides were mainly used for treatment. Most patients had a good prognosis, 38 cases were cured and 3 cases improved significantly. Conclusion Norwegian scabies is a serious skin disease caused by Sarcoptes scabies var. hominies, which is highly contagious and with a high misdiagnosis rate. For prevention and early diagnosis it is necessary to raise the awareness of the disease, carefully consult the medical history, familiarize with its clinical characteristics and perform pathogen examination timely.
