摘  要:目的探讨黄连素对耐碳青霉烯类肺炎克雷伯菌(CRKP)耐药性的体外逆转作用,为中药抗多重耐药菌感染的治疗提供实验数据。方法通过质粒接合转移实验证实耐药性R质粒的存在,采用微量肉汤稀释法检测黄连素对耐碳青霉烯类肺炎克雷伯菌实验菌株的最小抑菌浓度(MIC),并以此浓度进行质粒消除实验,以平板培养法确认R质粒消除并计算消除率。结果19株KPC基因型阳性的CRKP菌株,其中6株质粒接合转移试验阳性,占31.58%(6/19)。黄连素对耐碳青霉烯类肺炎克雷伯菌实验菌株的MIC为(1.38±0.72)mg/ml;质粒接合转移试验阳性菌株在24、48和72h的R质粒消除率分别为0、3.1%和4.7%;1%SDS在24、48和72h的R质粒消除率分别为1.1%、4.0%和8.1%。结论黄连素能较好地抑制耐碳青霉烯类肺炎克雷伯菌的生长,且黄连素对携带碳青霉烯酶基因的R质粒消除作用明显,从而具有逆转耐碳青霉烯类肺炎克雷伯菌耐药性的作用。Objective To evaluate the reversing effect of berberine on the resistance of carbapenem-resistant Klebsiella pneumoniae in vitro,and provide experimental data for Chinese herb medicine in treating multiple resistant bacteria.Methods The presence of R plasmids was verified by conjugative plasmid transfer experiment.The minimum inhibitory concentration(MIC)of berberine to the carbapenem-resistant Klebsiella pneumoniae was determined by broth dilution method,and the plasmid elimination experiment was conducted based on this concentration.The R plasmid elimination efficacy was determined by plate culture method.Results Among 19 strains of CRKP which had been characterized as KPC producing strain by genotype screening,6 strains were identified as positive strain of conjugation transfer test,accounting for 31.58%(6/19).The average minimum inhibitory concentration of berberine to carbapenemresistant Klebsiella pneumoniae experimental bacteria strain was(1.38±0.72)mg/ml.The R plasmid elimination rate of berberine on 6 test strains at 24h,48h and 72h was 0,3.1%and 4.7%,respectively.The R plasmid elimination rate of 1%SDS at 24h,48h and 72h was 1.1%,4.0%and 8.1%,respectively.Conclusion Berberine can inhibit the growth of carbapenem-resistant Klebsiella pneumoniae and reverse the resistance of carbapenem-resistant Klebsiella pneumoniae by eliminating R plasmid in which carbapenemases gene locates.
