局部应用普伐他汀干预激素性股骨头坏死的组织学观察

摘　　要：目的探讨局部应用普伐他汀干预激素性股骨头坏死的组织学变化。方法45只新西兰白兔建立激素性股骨头坏死模型后,随机分为3组,各15只。A组:模型组,不予特殊处理;B组:髓芯减压组,髓芯减压术后在减压通道内植入明胶海绵;C组:普伐他汀局部用药组,在股骨头髓芯减压通道内局部植入载普伐他汀的明胶海绵。分批在术后第4、8、12周处死白兔,进行股骨头组织学观察,同时计算各组的空骨陷窝率。结果组织学观察结果显示A组有明显的骨坏死,髓腔内有大量脂肪细胞增生,骨小梁稀疏;B组骨坏死面积稍减小,有较多脂肪细胞增生,骨小梁稍稀疏;C组骨坏死面积和髓腔内脂肪细胞数量明显减少,骨小梁密度基本正常,各时间点空骨陷窝率明显低于其他两组(P<0.05)。结论局部应用普伐他汀能促进激素性股骨头坏死的修复。Objective To evaluate the histological changes of steroid-induced femoral head necrosis intervened by topical use of pravastatin.Methods 45 New Zealand white rabbits were established animal models of steroidinduced femoral head necrosis,and then randomly divided them into group A,group B and group C.Each group included 15 rabbits.No treatment was performed in group A(model group).Core decompression of femoral head was performed and gelatin sponge was implanted in group B(Core decompression group).Core decompression of femoral head was performed and compound of pravastatin and gelatin sponge was implanted in group C(pravastatin group).The femoral heads were obtained at 4,8 and 12 weeks and observed with histology observation respectively.The rate of empty lacunae of femoral head was calculated in each group.Results Histology observation demonstrated that massive necrosis of femoral head was observed in group A,and a large number of fat cell proliferation was found in the bone marrow cavity,the bony trabeculae were sparse.In group B,the area of osteonecrosis reduced slightly,and many adipocytes were observed.At the same time,the bone trabecula became more sparse.Compared with group A and group B,the area of osteonecrosis and number of adipocytes were reduced,and the density of bone trabecula was nearly normal in group C,the rate of empty lacunae of group C obviously decreased than other groups at each time point.Conclusion Topical application of pravastatin can promote the repairment of steroidinduced femoral head necrosis.
