摘 要:目的探究经鼻高流量湿化氧疗(HFNC)在新型冠状病毒肺炎合并急性呼吸衰竭中的应用效果。方法选取湖北省黄冈医院2020年01月-2020年03月接收的72例新型冠状病毒肺炎合并急性呼吸衰竭患者,给予患者隔离、抗病毒、脏器保护、科学营养支持等措施。在此基础上按随机数字表将患者分为观察组(37例)与对照组(35例),对照组采用常规面罩吸氧,观察组采用经鼻高流量湿化氧疗。比较二组患者治疗前后相关指标,包括动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、呼吸频率(RR)、心率(HR)、呼吸困难改善和气管插管率情况。结果观察组使用经鼻高流量湿化氧疗后PaO2显著升高,PaCO2、RR、HR逐渐下降,临床症状逐渐消失,气管插管率明显低于对照组,治疗效果比较,差异有统计学意义(P<0.05)。结论经鼻高流量湿化氧疗在治疗新冠状病毒肺炎合并急性呼吸衰竭中有明显的治疗效果,具有临床中的研究与推广使用价值。Objective To explore the effective application and nursing effect of high flow nasal cannula in the treatment of acute respiratory failure combined with coronavirus pneumonia.Methods Seventy-two cases of coronavirus pneumonia complicated with acute respiratory failure received in Huanggang Hospital of Hubei from January 2020 to March 2020 were selected,among which 35 cases(control group)were treated with routine mask oxygen inhalation,and 37 cases(observation group)were treated with HFNC therapy.After treatment,the two groups were compared in terms of relevant indicators,including PaO2,PaCO2,HR and RR,oxygen partial pressure and carbon dioxide partial pressure.Results In the observation group,PaO2 increased significantly,PaCO2,RR,HR decreased gradually,clinical symptoms disappeared gradually,the intubation rate was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Transnasal high flow humidification oxygen therapy has obvious therapeutic effect in the treatment of new coronavirus pneumonia combined with acute respiratory failure,and has clinical research and promotion value.
