H型高血压合并缺血性脑卒中患者MTHFR C677T基因多态性的研究

摘　　要：目的探讨H型高血压合并缺血性脑卒中患者MTHFR C677T基因位点情况,分析其与Hcy和叶酸的相关性。方法选择2017年12月~2019年1月在沈阳医学院附属中心医院诊治的H型高血压合并缺血性脑卒中患者为A组34例,缺血性脑卒中患者为B组36例,同期无卒中及高血压病史、不伴随其他慢性病的健康者为C组32例,检测3组MTHFR C677T基因位点及Hcy和叶酸水平。结果 (1)A、B组与C组比较,基因型及等位基因频率差异均有统计学意义(均P<0.05);A组与B组比较差异无统计学意义(P>0.05)。(2)3组内比较TT型Hcy最高,3组间比较A组Hcy最高。(3)A、B组的Hcy与叶酸呈负相关,差异有统计学意义(P<0.05);C组Hcy与叶酸无相关性。结论H型高血压合并缺血性脑卒中患者MTHFR C677T基因具有多态性,基因突变可使Hcy升高;叶酸与Hcy呈负相关;对于H型高血压的患者,在降压的同时补充叶酸,可降低缺血性脑卒中的发病率。Objective To investigate the MTHFR C677 T gene locus in patients with H-type hypertension complicated with ischemic stroke, and to explore its correlation with the levels of Hcy and folic acid. Methods 34 patients with H-type hypertension complicated with ischemic stroke in group A, 36 patients with ischemic stroke in group B and 32 cases without the history of stroke and hypertension and other chronic diseases in group C were diagnosed and treated at the Central Hospital Affiliated to Shenyang Medical College from December 2017 to January 2019 were selected. The MTHFR C677 T gene locus and the levels of Hcy and folic acid were measured in three groups. Results(1)The differences of genotypes and allele frequencies between groups A, B and group C were statistically significant(both P<0.05);there was no significant difference between groups A and B(P>0.05).(2)The Hcy of TT type was the highest among the 3 groups, and the Hcy of group A was the highest among the 3 groups.(3)There was a negative correlation between Hcy and folic acid in groups A and B, and the difference was statistically significant(P<0.05);there was no correlation between Hcy and folic acid in group C. Conclusion The MTHFR C677 T gene in patients with H-type hypertension complicated with ischemic stroke is polymorphic, and mutations in the gene can increase Hcy. Folic acid is negatively correlated with Hcy. Folic acid can reduce the incidence of ischemic stroke in patients with H-type hypertension.
