腹腔镜微创治疗对胃良性疾病患者血清TNF-α、IL-6及术后并发症的影响

摘　　要：目的研究腹腔镜微创治疗对胃良性疾病患者血清肿瘤坏死因子(TNF-α)及白细胞介素-6(IL-6)及术后并发症的影响。方法选取上海交通大学医学院附属苏州九龙医院2016年1月至2019年1月收治的60例胃良性疾病患者作为研究对象以回顾性分析开展研究,按照手术方式不同分为对照组和治疗组,各30例。对照组患者给予常规开腹手术,治疗组患者给予腹腔镜微创手术。术后观察2组患者临床指标及并发症发生情况,比较术前及术后3、7 d 2组患者血清TNF-α、IL-6水平变化情况。结果与对照组比,治疗组平均术中出血量减少、平均肛门恢复排气时间缩短、平均住院天数减少(P<0.05);与术前比,术后3~7 d 2组IL-6、TNF-α水平均呈现先升高后降低趋势,且术后3、7 d治疗组均低于对照组(P<0.05);治疗组术后并发症发生率为10.00%,对照组为23.33%,2组比较无显著差异(P>0.05)。结论腹腔镜微创治疗胃良性疾病可有效减少术中出血量,缓解机体炎症因子水平,抑制炎症反应,促进患者预后恢复,且安全性良好。Objective To study the effect of laparoscopic minimally invasive treatment on serum TNF-α,IL-6 and postoperative complications in patients with benign gastric disease.Methods Totally 60 patients with benign gastric disease treated in our hospital from January 2016 to January 2019 were selected as the research subjects to conduct a retrospective analysis.The patients were divided into a control group and a treatment group according to different surgical methods,each with 30 patients.Patients in the control group received conventional laparotomy,and patients in the treatment group received laparoscopic minimally invasive surgery.The clinical indicators and complications of the two groups were observed after surgery,and the changes of serum TNF-α and IL-6 levels were compared between the two groups before and after surgery.Results Compared with the control group,the average blood loss in the treatment group was reduced,the average anal recovery time was shortened,and the average number of days of hospitalization was reduced(P<0.05).Compared with preoperative,serum levels of IL-6 and TNF-α increased in the two groups 3 days after surgery,which were lower in the treatment group than in the control group(P<0.05);compared with the before surgery,the levels of IL-6 and TNF-α in the two groups showed an increase first and then a decrease after 3~7 days,and the treatment group was lower than the control group on the 3 and 7 days after the operation(P<0.05);the incidence of postoperative complications was 10.00%in the treatment group and 23.33%in the control group,there was no significant difference between the two groups(P>0.05).Conclusions Laparoscopic minimally invasive treatment of benign gastric diseases can effectively reduce the amount of intraoperative bleeding,relieve the body's inflammatory factor levels,decrease the inflammatory response,accelerate the recovery of patients'prognosis.
